In Medieval: Down, Up, Square, Triangle, Triangle, Circle, Down, Up, Square, Triangle. It opens up the smaller cheat menu, but I liked it because it spells out the phrase Dust To Dust.
In Tomb Raider II: Level Skip/Lara Explodes cheat, mainly because I never knew the difference between the two and often exploded when I was trying to get to Venice.
Also there was one in Shinobi where you went to options, changed your Shurikens to 00 and you held a button for a while and it turned into an infinity sign. Pretty cool . Not really a code though...
RTS games have really awesome cheat codes, though they're not necessarily potent.
"O CANADA" in Age of Mythology gives you a Lazer Bear. He wears a Canadian flag, has the line of sight of a tower, 99/99 armor, regenerates health, has 120 pierce attack, and moves about ten times faster than anything else in the game.
"where's the axe" or something along those lines in Age of Empires III spawns a giant bust of George Washington. He bounces across the map and fires laser beams out of his eyes.
The best cheat name? "i love the monkey head" in AoE II gives you a VDML man. Runs really fast, but otherwise bloody useless.
