CoD 4 multiplayer, while carrying the G3 my finger slipped and I accidently discharged a round. The shot went through the wall and embedded itself in some unfortunate enemy's skull.

used a UAV with enhanced warheads to knock down the building around the first Bravo objective on Nelson Harbour and killed 9 guys. 7 from "Destruction 2.0" and 2 from the last missile.
then later in the match i "Airkilled" 4 guys in a singe ramming run with the same UAV from the start of the game... god i love UAV's.

In MW2 I about to die so I chucked a semtex grenade and it happened to land on the bomb that the enemy was trying to defuse (I was playing Demolition).

Me and my friend were playing Star Wars Battlefront, and I was being stupid and trying to snipe people with a rocket launcher. I shoot and right as it's about to connect with this guy who hadn't even noticed it was coming and my friend, who had just became a jedi like 5 seconds ago, runs by and gets hit straight in the face with my rocket and dies. It was awesome.

Well I sniped a chopper pilot out of his chopper in Bad Company last night, causing it to crash...

in bioshock on survivor when you go to point prometheus for the second lot 142 dosage i had no health packs half health and 4 eve hypos and 46 napalm that was IT. I just said fuck all splicers fuck all turrets and just ran electrobolted the turrets and escaped unharmed.
A friend of mine has a special talent for low probability deaths. While playing Fallout Tactics, he gets crit'd and dies in the first level... not bad so far, that happens. Then he tries again, and gets crit'd and dies, SIX MORE TIMES in the first level; in nearly the same damn spot. He then gave up on the game.
He was also killed by a rabbit in Asheron's Call... Rabbits are significantly weaker than any 1st level character, and he was level 10...
For me, I've had one I thought was even worse than one-in-a-million, but that was before; I now know the AK in Counter Strike is basically the "random bullshit" gun. With that gun, I once killed 3 counter-terrorists with a single 3-shot burst, all three were head shots, and all four of us were running at the time. Then, just to prove it was dumb luck, I promptly died to the 4th CT to run around the corner... without hitting him.

While playing Resident Evil 5 on that section where you need to stay on the spinning platform and shoot zombies as they try to kill you, I pulled out my sniper rifle, shot one of them whereupon the bullet proceeded to exit the zombie and hit the emblem behind him that I didn't even know was there.
Perhaps this doesn't count as it was not I that did it, but I got shot down by an Abrams tank while flying the Hind attack helicopter in Bad Company 2.
Same game, Arica Harbor. I fire off a random tank shell into the distance at the very start of the game, I hit a Humvee with 4 people. In that same game, I managed to hit a speeding quad bike with the M2 Carl Gustav. Granted, I saw him coming and just took a guess and fired, but still, the amount of flying about those things do I thought it was absolutely hilarious.

Completely surprised and also scared as hell (was 14 back then) i jumped and pressed the left mouse button. After a brief and kinda pathetic "BIP" of the silencer the terrorist was lying dead on the other fucking side of the map...

Not so much as one in a million, but a hilarious strike down. On TF2 a Demoman and a medic were camping my spawn on gravelpit. I ran out their as soldier and killed both of them... with the shovel.
In L4D2 I was lining up my shotgun for a nice Witch headshot when the horde arrives. I fire at the Witch, hoping to kill her before the rest of the infected start attacking me. My first shot misses though and has the lovely effect of pissing her off. So now I surrounded by a bunch of infected fucking up my aim and making it impossible to move anywhere with an angry Witch about to rape me. I fire again and despite the fact that my aim is totally off due to the surrounding Infected wacking me, I somehow manage to one shot the Witch in the head with a chrome shotgun.
Damn was my heart pounding after that.

in the desert map with the gunships on each side at the very start I jumped in an abrams and just lobbed a shell as far as I could and managed to hit the enemy chopper as 2 guys got in it
and in the rush map in the snow, harbor something, I was playing recon and was having a pistol duel with another recon, we had just both reloaded our clips and so I went around a corner and took 4 shots and managed to head shot a medic that was behind him that I didnt see then head shot him and killed an assault that was coming up behind me when I turned around

In GTA IV. Took the stupid cousin to watch some "american titties". Wrecked my Comet on our way to the club so had to steal a bike.
Just right next to the club there's a parking lot, always full. Trying to make it quick, I rush into the parking lot, miscalculate the speed, the momentum, the space and everything else. My bike rams the parked car, Niko stays in the saddle and the fat bastard flies just right into the strip club's door.
