are there any of these in existence? the only one i can think of is mount and blade...
edit: medieval as in no fantasy elements

Mount and Blade comes to mind. It's a small indie game and fairly unpolished, but the combat is fun and the many mods will enhance that quality. Be warned though, it has absolutely no linear plot to follow. The site is www.taleworlds.com

It seems RPG makers don't like medieval times without adding a dragon or two and a few elves...

I love Mount & Blade really bad, but when I think about it, there aren't many games like it... Very few actually...

Mount and Blade is it. You want a historical Medieval RPG fix, you just have to play Medieval II: Total War and zoom in really close during the 3D RTS battles.

It's a shame, dude. A real effing shame.

There was this old early 90s rpg called Darklands. You play some kind of German knight in the 15th century open world and all that jazz. It was actually pretty fun...

Well it actually is in a bit, you control one guy that you control through some sort of a story mode, that's an RPG element right there.
...no, it's really not in the least bit. It's narrative, not RPG and just because narrative is a focal and standard part of RPGs, it does not make Assassin's Creed one.
And I wasn't saying it was one, I was saying it had an RPG "bit" in it. So we agree, it's not an RPG but has the narrative.
