Doing a lot of martial arts most of the piercing stuff is out of the question. Im currently quite attractive and in good shape so for the moment I'll rule out plastic surgery. I don't find scarrification attractive. All of my scars have a story more involved than "I paid some guy to do it".
I quite like tattoos. If money was no object I would have a single sleave on my right arm. Unfortunately the price of tattoos is extortionate so I'll give that a miss.
That leaves me at level 1 I guess. I have no real urge to shout "look at me, i'm different" and try to upset my parents.
Now if in the future there was some sort of cyberpunkesque performance enhancing muscle and bone graft you could sign me up straight away.

Can't really vote in this, since the option I would choose isn't there.
I would never modify my body in order to "beautify" it's appearance (like face-lift, penis enlargement, face scupting etc.). Also I don't see the point with pushing metal objects through my skin and leaving them there (piercing). I don't see any reason for sliting my own tounge in half.
But if the technology would be available, I would, without hesitation have my own arm chopped off if it could be replaced with a cybernetic arm superior in strength and dexterity to my own arm. That sort of body modification isn't available yet, but when the day comes, I will gladly begin my transhumanistic voyage.
My body is just a vessel after all. If it can be changed in favor of an improved mechanical vessel, then why not?

I'm not into any of this stuff. I think people in general look better without it.

Level 0 here, Piercings and modifications like that are disgusting, 75% of tatoos are terribly ugly or just plain ugh; I can't stand stuff like this.
Level 1. I see no point in modifying my body above shaving, showering, and other general hygiene stuff.

it really bugs me when i find someone thats generally a cool person, and very attractive...and BAM has some weird body alteration...i dont get it, i can understand plastic surgery to take care of that birthmark, or to change the way you look because you had a crooked nose or something, but why change your body to look worse? -I DONT GET IT-
Level 8. Not that I'd get any done to myself, what other people do to their bodies is hardly any of my concern, and as long as they're completely aware of the benefits and consquences their decision shouldn't be too stupid.
