Second from hardest for first playthoughs unless there are only a few options then I'll take the highest if the game is good enough to replay.

I always start with Hard because I don't want to be disappointed... often I have to crank it down to Normal for a first play, but it's more fun to be challenged right away.

Normal or hard. Depends on the type of game really. I usually end up playing through all the difficulties

I usually go for the hardest one. If a game doesn't let you choose the hardest one until you unlock it, I'll play on easy then switch to hardest.

I used to play with cheats on, when I was actually scared of getting killed in games. Now I'm a bit calmer (read: I manned up) so flick it on to hard to start :).
Cookie for being honest. Only games I have played with cheats on is probably the GTA series only becuz Im too lazy to go buy guns.

Very first run through of a game I select the second hardest difficulty if there are four or more settings. If there is only easy, normal and hard then I'll select hard. This is usually just to prolong the life span of the game for me, as I breeze through games way to quickly.

Depends, again, on the genre. I start most shooters on Hard, and if it's a sequel I usually start it on Hardest (if that's available).

New games, especially from genres in which difficulty is harder to spot I usually start on Normal. I remember Dragon Age: Origins being pretty damn tough on Normal. KotOR however was pretty easy on Normal. RPG's often all over the place, their difficulty is pretty hard to detect.

The only game I can recall I started on the lowest difficulty setting is Ninja Gaiden 2. And I still haven't finished it.
*Dammit Shouldve spotted this before I went on to quote another person*

Anyway, agreed with having difficulty spotting the diffculty that is right with RPGs. I played DA:O at normal difficulty and was actually struggling at some points *Friggin dwarf quest*. And with shooters its alot easier (especially with the COD series).

Ive got the first Ninja Gaiden and have not completed it and will never.

If it's a genre I have some experience with, like FPS or TPS, I'll select Normal mode. If I don't have much experience with the genre (Stealth & Racing games, for example) I'll usually start with the Easy mode and see how that goes. For horror games I usually select the most difficult option available right from the start though, as games just aren't scary if you can gun down the enemies with casual ease.

I always play on Normal. Because I have this idea that that's the difficulty the game designers intended the game to have.

I usually start on the one under the top (unless, of cause, there's only two). Once I finish a game once, I'm not normally as eager to play it again, so I don't want to be able to rush through it without any challenge. But then, if I enjoy it and DO want to play it again, it's nice to have someplace to go.

Oh, but pretty much agree with xI Kinshasa Ix. If I expect to play a horror game from beginning to end, I'd consider going hardest to get the most immersive experience out of the combat (all the better if it has different settings for combat and challenges, ie Silent Hill 3).

Always start on normal and move up if I find it to easy.
But in a lot of games hard just means enemy can take more damage and that is not harder in my mind, it's just more tedious and time consuming.

Mostly normal, but it really depends on the game. I suck at some game genres I love, so I often turn down the difficulty there. I seldom start a game on hard, unless it really is too easy. I don't play for a challenge, I play for amusement.

I start on easy to get used to the game, then switch to medium/normal. When I feel that I'm good enough at the game, I change to hard.

Typically normal. I did play Halo 3 in Heroic mode after years of not playing any Halo game because the controls make so much sense and are easy to remember.
I like to pick one that has a balance between challenging boses that require strategy and slightly easy grunts that you can have fun experimenting fighting moves with. Struggling to stay alive and having to replay a level over and over again are no fun to me.
