Was in superdrug in the make up section with my girlfriend and a shop assistant came up to us and said "would you like any help ladies"? I really didn't care as we were both buying new make up :)

I was down at a camp in middle school, my voice hadn't changed yet and I had long hair.
I had been hanging out with the same girl for almost the whole week, I heard her refer to me as "she"

Only once, when I was sporting a ponytail. Confused me though, as I'm short and stockish, with proportionally very broad shoulders and no hips to speak of.

Guess the old lady couldn't see past the hair (which, I shit you not, was coo-ed over by the whole hairdressers as 'luscious' when I had it cut off).

Never, which at first thought surprises me a little, but the more I think about it I realise despite not being particularly manly, I'm not particularly feminine either.

Oh god yes, it happens all the fucking time but it always happens on the phone. it doesn't matter how deep I make my voice they still ask "Hello Mrs. (my last name)" and I always think "Mother fucker" and then I hang up on them because no matter who they are if they mistake me for a woman I don't care what your business is.

I was once, by a very old man. You might even call him ancient. I had long hair. It was the shiny clean, advert kind of hair. So Methusalem wants to ask the way, and walks up to my back and asks where he needs to go.

After explaining his to him in my tenor voice, he says thanks, young lady and moves on.

I was baffled. mostly because I had a beard. A thick one t that.

I've been confused for a lady twice.

The first time I was very small and had long hair. I'm not sure that counts considering my age at the time.

The second time was at the age of 20. I went to Kroger after seeing the Rocky Horror Picture Show. I was still wearing my Magenta outfit. Some guy seeing me in a maid outfit from behind thought I had a cute butt and sexy legs. I blushed and continued walking while my best friend laughed mercilessly. On the way out, the guy saw me from the front and his surprise was evident.

Yes. Magenta is also known as "the domestic" in Rocky Horror Picture Show. Going as Magenta means wearing a maid's outfit. I figure if you go to a live showing of Rocky Horror and you don't wear something interesting, you are an asshole. Yes, I am directing this at all the guys who think they are "dressed" for Rocky Horror by wearing a conservative, beginning of the film, Brad Majors outfit.

So... yeah... short heels, hosiery, shirt style corset, garter belt, panties, lower-mid thigh length skirt, maid's apron, maid's cap, and a feather duster... I couldn't recreate exactly what Magenta wears so I did my best visiting thrift shops and accepting gifts from female friends who wanted to see me in the completed outfit.

I also shaved... everywhere...

So yeah. I had some sexy gams that night and the guy let me know. A comment he regretted upon seeing my face and chest.

Nah, long chin, large ears and nose, broad shoulders. Not really. Asked if gay, but never mistaken for a female.

EDIT: The reason I was asked if I was gay is because I was "Acting feminine" (I can't remember how. I think I hugged a male friend) and my voice hadn't broken at the time, so my voice was quite high pitched.
That's why I mentioned it.

Ive been mistaken from behind as a girl because of my mid-long hair, then Ive had 3 people mistake this same girl for me, im still trying to find who this female look alike is xD

All the time over the phone. I am very soft spoken, and so when I talk to people on the telephone, I get called "ma'am". It's really annoying.

EDIT: A blind man called me sweetheart once becasue of this as well.
