Oblivion, the Dark Brotherhood quest where... well, you know the one.
I felt slightly bad about...

Killing Antoinetta Marie.

I've always had a liking for psychotic characters, especially ones who can act all sweet and innocent right up until the point where, suddenly, they're not. Antoinetta was like that. Plus, though it's slightly odd to say this, but she was pretty attractive for an Oblivion character. Bearing in mind it's only a game, of course.

That said, I also felt pretty bad about Lucien Lachance's fate too... though I was pretty pleased at getting a sweet horse out of it :D.

Yes, like in Mass Effect 2 when I got Mordin and Thane killed in the final mission, or in Dragon Age: Origins when Morrigan left, and lots of other times. Mostly its in RPGs when you get this sense of guilt, and it's an odd feeling to get while playing a game, but it's still quite good for games to be able to do this.

GTA:4
I immersed myself in the game, and that wedding drove me to power through the remaining story, and then once the happiness island shootout was done, I felt a little guilty.

Kate was dead, but at least I still had cousin roman, but the chance to really settle down and leave the past fully was gone to me.

When I did know what I was doing beforehand:

The KotOR series quite frequently. I would very occasionally start a good character, but he would turn into a malicious bastard as the good became less fun, and I sort of feel guilty due to the immersion of those games (if I start evil I don't feel bad though).

When I did not know what I was doing:

When I found out in the Fighters Guild questline in Oblivion that

the Blackwood Company had tricked me into murdering a village full of women and children while drugged

Funny thing is, I murdered a whole village of civilians of my own accord in that game and was unnaffected. All in the presentation and surprise.
