I don't do MP too much, but I usually mute all players. I'm just there to shoot guys, not to interact with some of the more "vocal" community members.

Only once, some dickbags starting talking shit to people with headsets...they were just awful...it wasn't just friendly trash talk...it was actual hate speech...and once on WoW I had just another awful tank who was such a shit head...overall there's some assholes...thankfully not all of them have ways of talking that can get to ya
I played CoD4, Halo 3, CoD3, Gears of War, and other games on my Xbox 360 and only ran into an asshole about once every 10 matches. The problem isn't that big.
Now I play on PC, and it seems there's more of a scale. You get Minecraft, with hardly any assholes except the griefers. In fact, many people will go around giving things out for free just to help new players. Then you go down a bit to Bad Company 2. People don't even talk in that game. Next comes Black Ops. People will sometimes say NOOB, but that's about it. But more than Bad Company 2. And then there's Counterstrike: Source, where every second person is a screaming lunatic high on pot.
Oh, and there's also Team Fortress 2 which takes the two extremes and puts them together. You can't be in the middle if you play that game.

Depends on the game. Online shooters aren't really my bag, outside Left 4 Dead, so I don't run into much there. Games like Dungeon Fighter Online, though, I do from time to time. Still kind of rare.
Fable 3 has been working well due to the one on one co-op and the fact that anything you do only affects your game...
I think most annoying people go for shooters (not that there's anything wrong with shooters), there is a developing trend for me to mute/avoid/filecomplaint every time someone pisses me off, but I feel like a prick due to the fact many times it's unintentional.
EG: Me and some friends in chat, one of my friends speaks fluent english and german, german-speaking friend of his jumps in, germ-friend keeps talking RREEAALLYY loudly so I ask him to stop shouting (I have very basic german), he ignores me so I ask again, he then begins hurling abuse in broken english (which indicated that he could have talked to us but dident), calls me racist at which point I mute and boot him, and then I feel bad because I probably came off as the bad guy...

Rarely, extremely so. I would have said "never," but it has happened a couple of times, so it's not quite never. I don't know about Xbox Live, but griefers are extremely rare on the PC, at least in my experience.
i AM the annoying person. just to have a laugh i put on a really nerdy voice and complain about them "violating the code of conduct".

I never go in specifically to annoy people. I do take retaliation to the absolute extreme in games like RDR. If I'm just riding about minding my own business, and some idiot kills me for no reason, I will spend as long as it takes until they leave the server just dogging and killing them over and over again.
I can say rarely because I've into enough aggravating people online that I generally just avoid the multi-player or PvP segments entirely.
Very, very often. One of my friends has a whole clan of people on his friends list who annoy the crap out of me, and so I don't get to talk to him that much because they just yap in my ear.
tbh i run into people alot. usually its people that are just annoying, dick heads on the mic or team killers, but ive started to run into more bad team mates, you know the guys who dont really help you and screw you over for their score.
im also no running into people who are just terrible at the games (or iv gotten really good and i just need to up the difficulty) but now after buying red orchestra ive notices people are annpyingly good.
