I just findshed replaying PROTOTYPE again when I looked back on the games I've played, movies I've watched, and books I've read that involve some sort of deadly virus. And I thought to myself, "which is worst?" So to the point. What virus or disease from any form of storytelling, be it a game, a book, a movie, a comic strip, a zoetrope, anything, is the worst of the worst?
Personally my vote goes either to the Blacklight Virus varient DX 1118 A.
Engineered to copy the best biological traits from a host, and instill them in the carrier, when introduced into a host has a delayed kill ratio of 99.999%. When no host is avaliable, the disease can manifest itself in an artifical body several times bigger and more powerful than any human.

it's called NDM-1 (pretty cool name) and it's super resistent to antibiotics, and it causes very bad infections before killing you.

My vote is for the F.E.V. It is special, because unlike a vampire (kind of bad ass, in some ways), a mindless zombie, or dead, the F.E.V. turns you into a giant, androgynous savage with some remnant of former sapience. I think that of the other options, the F.E.V. would be the most degrading to you and most horrifying to the people who knew you, that is unless you were already a dim-witted savage. In that case it wouldn't be so different.
My money's on FEV as well---that stuff's some seriously nasty juju...and to hear Fawkes tell it, the struggle to maintain sanity as the changes occur is the worst manner of torture upon one's mind.

I'm not dissing op, I'm just saying that you've quite a bit of zombie knowledge as per the poll question. Resident Evil being such a dominant zombie gaming figure for so many years makes it strange to me that it's not known by the poll creator.
While it's not in any media that I'm aware of, I submit weaponized Spanish Flu. It killed 3% of the worlds population in 1917 and 1918, back when the world's population was only estimated at 1.6 billion.
500 million people were infected worldwide, and between 50-100 million died. Let some genetic engineers get at that and it'll be worse than any virus outbreak anyone could imagine.
Only virii I can think of that'd be as bad or worse would be ebola, which already has a 50-89% mortality rate without genetic boosting.

Besides, Super Mutants are morons, they go for the biggest thing resembling a weapon they can and don't take cover, while going head on against one plays to it's strengths, outsmarting it by using proper weapons, cover and strategy makes them a piece of cake to kill.
The danger of a virus lies in it's ability to spread. Sure, 28 days later zombies change in 10 seconds, but it's easy to keep contained. L4D Zombies appear to be the most deadly, or any airborne virus (I am Legend). To keep things fair, I would actually leave out airborne viruses from the competition because they are too powerful. Excluding Airborne, the Prototype virus has the greatest potential because of its high mutation rate, and the possibility of super-super powerful infected (like the protagonist).
