Rygar - This one I'm perhaps most proud of, it's a tough game that ruined me when I was a child. So I felt the need to get my revenge. This one can be done in about 30 minutes.
Wow. I still remember the first time I beat that game. I can't imagine beating it in 30 minutes, much less without dying.

My first death-free victory was Dragon Warrior. I did have to save since it ate time like nothing else, but I never had to read "Thou art dead."

Prince of Persia! ;-)

And being serious now - I managed to beat normal mode on Splinter Cell: Conviction without dying once.

Not on my first try, but Imperishable Night. On Normal.

You know, this sort of thing.

Maddening, I tell you.

Note: This includes the ridiculous "last spells" at the end. You know, where she fires off a bullet pattern, vanishes once you beat it or die, then repeats the process after reappearing. If I was just trying to finish the game with no deaths (just to clarify, I did beat the game with no deaths, not just stage 6, though I DID use bombs, no slow mode before you ask) it would have been a good deal easier.

Edit: I suppose every roguelike counts too. I've beaten Nethack, ADOM and Dungeon Crawl Stone Soup. You know, eventually. I died plenty of times, but since you can't save and you start over whenever you die, it probably sorta-kinda-half-counts.

Alex Kidd in Miracle World. So much Janken, though I did drop two lives in my best run through. my brother however played through three times in a row without dropping a life.

I've completed Torchlight on hardcore, which means you can't die or your save is "deleted". It was only on normal mode though, plan to do hard hardcore when Torchlight 2 is around the corner.

My goodness. I thought I had achieved something with my (now I see) pitiful dead-free stories of game grandeur, but there are some really (for lack of a better word) spooky gamers out there. :-D
E.G. An 80th level Death Knight without dying??? Ironic and achivementfulous no doubt.

A couple of persons mentioned beating "Portal". I remember that I was almost there. Feeling that my cake was just around the corner, and then, under a mad whim I went close to some edge to gaze at the niceness of the labour of the artists of the game, misjudged a pixel and died.
I almost cried. If anyone here has ever seen the japanese-polish film "Avalon" [http://en.wikipedia.org/wiki/Avalon_(2001_film)] (that you should if you have not) you know exactly how I feel.
Didn't touch the game for a few mournful days 'till the shock subsided, then I discovered that I was not that close anyhow, and that there was no... you know.
