Yes. Let's hook you up to a polygraph test. There is one type of game that I will always play on Easy, and that is games that are known for being hard.
Fine with me - the simple fact you think games this gen (with the exception of a couple) are "hard" is laughable, in terms of desgin not many games are difficult - some can be annoying due to the sheer number of flaws (poor cameras etc..) but not many games are actually difficult on their highest setting, just because Onyx Oblivion finds games difficult dosn't mean everyone does.

I voted hardest because thats what I play....Gears of War on Insane, COD 4 on Vetren, Halo 3 on Legendary (Disk broke after cortana) are a few examples of games with a difficulty setting I actially completed(EDIT: Except for Halo 3) (most of the games on my GT are rented)

I go with medium. I don't have all the time in the world to play, so a 'semi-easy/tough' challenge is enough for me to be entertained. If I was younger, I'd be all over the 'super-insane-o' difficulty.

Easiest for the first run. I see no reason to give myself a hard time, though if I really like the game, I might try to challange myself if I replay it.

I go for the medium difficulty. It's usually enough to prove a small challenge whilst making me feel awesome for getting through the game. If a game has any replay value i'll want to play it on harder setting the second time around.
If there's no incentive for a second playthrough, it'll most likely gather dust on my shelf.

I'll go for whatever is comfortable. Often I'll go for the easiest setting to test it out, find it's too easy and then ramp it up as high as necessary to allow successful completion without being bored into a coma.

Typically on medium, but if its a hyper-realistic tactical game (like the old Rainbow Six games), I crank it to "realistic."

i do easy first 2 get a feel of the game, medium to find the hidden stuff, and hard only if there are any throphys for it and even then i sumtimes dont do hard. Saves me money so i dont have to buy another $50 controller when it gets thrown.

ill go for whatever the gameplay or immersion asks me to set it to. for any generic shooter where the plot, narrative and gameplay is good enough, i'd hit easy. for specific shooters where difficulty actually matters more, i'll go for the hardest (see: delta difficulty in crysis, real difficulty in full spectrum warrior/ten hammers).

combat for me is pure filler, i get nothing out of it unless it's somehow related to other gameplay mechanics or story.

in any given multiplayer FPS, i'll finish in the top tier (1 through 3).

I'll play through a game on medium/normal settings because in my mind, that is how the designers meant for the game to be played. Normal means everything is how they intended the game to be played, all the other settings are to fine tune the difficulty of a game for the player. If a game draws me in, I'll kick things up to hard or very hard, in order to test myself and to make the game more enjoyable.

Why choose? Why not have adaptable enemies?

It's easier for me and as soon as I get the swing of a game I don't have to make a conscious decision, the game does.

Other than that I play the difficulty where the enemies just say 'Hey! I like bullets!' so I can just run off and explore and get to the punching-bags with legs later.

Also if you want a challenge in Halo take on your own marines, they want to kill you more than the Covenant do...

I always start on the most difficult so I don't end up developing skills that won't help me in the harder ones (like runnin and gunnin)

Depending on the content of the game I will change the settings. If it's a detailed rpg with lots of things to see and do I'll play it once on the intended difficulty. Then if I feel like replaying it, I'll turn it on hard. For Mass Effect I played the game on easy, normal, then difficult; Bioware seems to want you to do that, judging by their achievements.

For FPS and RTS I almost always start on easy mode, because I know I'll be coming back to, and playing them again. Farcry 2 was played on easy mode once, and I am sure that as soon as I brave the 15 min intro scene on rails again, I'll play it on normal.
