Just Normal. I don't want it to be frustratingly hard, but I hardly want a cakewalk either. Normal just seems like the best one.

I pretty much always do the first run on normal, then go up to the harder difficulties.

However, if I'm borrowing or renting a game, I'll take it down to lower difficulties to save time.

Depends.

If the game has an Achievement or something for beating it on a certain setting and I feel like getting it on the first run, though only if it's easy to acquire.

Depends, really. Though, I tend to set it to the easiest first, then murder myself on the highest setting. :p

Normal for the first play-through. Every time. Easy always seemed like a cop-out.

If I DO want to do a second play, that's when I go for difficulty.

I'll play the toughest setting, but usually I'll stop one away

unless you're talking about Rock Band

I always play on Normal or equivalent for my first play-through. Then I set the difficulty as high as possible for the second, because I like a challenge, and I'm an Achievement whore (and a lot of achievements on the Xbox 360 are specifically for the highest difficulty, I've noticed... :p).

Start with "standard" or "medium", just to see what the game designers had in mind for the game. If the game feels too hard or easy I change the difficulty setting up or down. I usually only have to adjust a game's difficulty settings once, though. The main thing to remember is that I'm here for the experience, and if the difficulty settings are SNAFU then the game goes to the waste bin faster than you can blink your eyes.

Easy if im starting out, normal if I know a little bit about how to play and hard if I have played the game before.

On a four tier scale, I usually go for difficulty three. On a three tier scale, I'll put it on hard and see how it goes. But I absolutely hate it when a game has an unlockable difficulty but keeps it secret from you.

Normal, because I feel as though that is the difficulty at which the developer wants me to play the game and the experiences are based upon. It's all fine and good to appreciate a story, but when you're playing a game, an interactive medium, you have to strive for more than just story. In order for a game to truly be considered great, it has to achieve a healthy mix of game story plus great gameplay.

nearly always play easy or whatever at first, then if i really like the game or beat it once, i'll crank it up a few notches, i hate playing games where you die 20x on one part (especially when that part SHOULDN'T BE IN THE GAME! (such as driving parts in fps where your car blows up on anything)

so usually "heroic" or "above normal but not insane piss hard"

Easiest possible. The sad thing is that while playing my brother-in-law's Street Fighter, I couldn't get past the third fight, even on easiest.

In a largely story driven game (e.g. Mass Effect, Dragon Age), I don't want to be held up by combat, so will generally go for the ridiculously easy settings so I can spend more time talking to people.

Shooters etc... I tend to hit at a hard difficulty. Typically the one above normal, but below masochistic/the game will actively go out of its way to screw you over
