If on steam, suspension of the account, including unrelated games for the first offence, then if done again, permanent loss of the steam account.

30 - 90 day ban for a first offense. Permanent banning for the second. Unless the game runs on dedicated player owned servers in which case perma-ban for the first offense is entirely acceptable.

Go to ban. Go directly to ban. Do not pass go, do not collect 200 gamerscore.

Label them as a cheater, light ban and then after words for 1 month every weapon they use will make a pew sound, and they will be forced to do little damage.

I remember I once read that the only correct response to cheating should be the console shooting our your disc like a skeet aimed at the cheaters neck.
oh god..

PULL!

edit:

OT: i think that they should have a long ass ban, so a month to a month and a half, then if they do it again a perma ban for the game and (if they are on steam/xbox live/psn) they should be labeled a cheater on that account forever.

Can someone help me understand how a 24-72 hour ban solves anything? It's no better than a warning. You KNOW the person is going to do it again if all they get is a tiny slap on the wrist. I'm a big believer in making an example of someone.

I like the idea of labeling them as a cheater for everyone to mock and ban from their server, but that doesn't really work for console games without dedicated servers. Perhaps the first offense is a 2 week suspension and having your gamertag replaced by 'CHEATER!' for 6 months, and second offense is a perma ban? Seems much more reasonable. I guarantee you that if someone is only suspended for 3 days or less and no other punishment is dealt, they will be back to cheating before the end of the week. A cheater is always going to cheat, just as a pirate is always going to steal. It's in their douchey DNA.

Insta-ban, but include a brief warning at the start of every game informing players that any kind of cheating, modification or exploitation in multiplayer will lead to an immediate ban. Include the warning so no one can complain they were unfairly punished or weren't aware of the ramifications of cheating.

The cheater should be made to climb a tall, thorny tree in the nude, whilst angry birds peck at them from above and attendants prod them to climb ever higher with spears below. Additionally, it should be freezing while this goes on, with someone regularly pelting the climber with a hard snow-ball.

Ban them from all servers except special servers just for them where everyone cheats, just for laughs.

Depends on the scenario at hand. If you have, say, a private server, then go right ahead and cheat all you want. You own the server, or know the guy who does and he trusts you enough to play on it, so clearly you have every right to do whatever you want on that server. People joining these servers have no right to complain: They can find a different server, they continue on this one knowing full well the consequences.

On other people's servers, such as publicly hosted servers, cheating is a no-no. Cheating simply to win is just plain moronic. Cheating to have fun is less moronic, but do it on your own time.

Points summed up: Cheating privately is fine, cheating publicly is bad. Unless is really hilarious, in which case the other players should just loosen up and join in the fun.

Why not make the game cheat-proof in the first place? The harsher the punishment, the louder those punished due to mistakes will cry out, and the louder those pretending to be innocent will whine. (see the autistic brat and his mom that was in the news a little while ago). Just make the game properly, so only people going through some serious effort can cheat. (which will keep it rare).

For those calling better players "cheaters", or ranting online on how some game is full of cheaters no one except for them seems to encounter, since those are the far more frequent, and frankly, more annoying offenders, I propose a fair warning the first time, and the second time a permanent life-long ban from every source of entertainment except for bookstores.
