Thankfully, never had any of that.

But I'd say anything attacking the nerve centers, which directly attack the pain center.
I once had anesthesia injected directly into my nerve and nurses had to hold me down, cause I was writhing in pain.

Testicular pain is the only gender exclusive one that i know of, so the rest are disqualified on principle of the qualifier "worst male pain". I hear having your vas deferens twisted around each-other is insanely painful though.

I voted kidney stone as the most painful because out of everything I have experienced that was the worst and the options to deal with them are limited, and frequently result in people simply passing them painfully.

I voted for kick in the groin.

Mainly because of the fact that one single kick may result in impotence.

I can honestly say that a kidney stone hurts a lot. For a guy the pain is debilitating. When I had one, I was in so much pain I threw up anything I ingested...even water. Passing the damn thing isn't' really painful at all but when that sucker is moving around there is nothing you can do to quell the pain. Only time it didn't hurt too much was when I got a morphine drip at the hospital even though I could still feel some of the pain.

It was the worst I have ever felt in my life.

100% positive kidney stones. i have passed 14 in my life. i have a biological predisposition where i dont process some minerals correctly and have them about once evry 6 to 9 months. the first ones i ever passed the nurse said "i had twins then i had kidney stones a month later... id rather be forced to have twins again once a week for the rest of my life".....i think this was supposed to make me feel better. it didnt. imagine a pickaxe... wrapped in barbed wire... on fire. being swung into your side at a descending angle and the tip of it just barely passing out the end of your urethra with each swing... now imagine no amount of breathing, moving, running, screaming, crying, or passing out, can help you escape this pain. now call 4 different anestheticians to your room. have them all try different solutions and finally after four hours (your still conscious and screaming) they decide to hit you with a dellodad and morphine combo reserved for terminal cancer patients, you immediately vomit, and go instantly unconscious, and awake to the nurse shaking what looks like a 15 millimeter lumpy marble in your face, but its actually the stone. your penis at this point feels.....lets say uncomfortable, but you honestly dont care, because the process of the stone traveling out of the kidney down the ureter to the bladder is so excruciating that you wouldnt mind if you woke up dickless as long as you woke up pain free. at least thats my experience ( oh and ive also been through everything else ive read on this list. so i have a frame of reference).......

edit. ok i didnt read closely enough. whoever got their testes twisted to the point that they lost them or were going to and had to be knocked unconscious by the doc.......yea. you win. or whoever that was that you witnessed with it.. wins.

@person above who is passing a stone.. trust me once it moves into the bladder there will be a ton of relief for you, passing it is nowhere NEAR as agonizing as the movement from the kidney to the bladder. it will seem almost calming when you feel it start to uncomfortably move out of your urethra, because at least your entire insides wont be on fire anymore.

Nothing tops Johnny Cage's Nutbuster X-ray.

He's not anatomically correct so that would have felt the same as getting punched in the arm lol.

Renal colic is often called the worse pain. Women who've had it, often say it's worse than pregnancy.

I had a kidney stone once. Wasn't that bad, to be honest... At first I thought "Oh shit, I think I've got a minor infection of the dickus" but yeh.
Depends on the size of the stone, I'd say you were lucky and got a small one.

I'll say this: http://en.wikipedia.org/wiki/Trigeminal_neuralgia

because I have it and have never felt anything so painful by comparison.

I haven't had kidney stones or experienced a zipper problem so I'm not sure on those two, but I have suffered several injuries on that list and getting hit in the grion is the most painful out of all of them
[spolier]followed by women >.<

jk women have never been painful to me[/spolier]
but seriously though, I've had back pain that has been so bad that moving was extremely painful and not moving was only less painful but since the question asks to take time into consideration the grion is the point that causes the most pain.

I once paused for a pee break while cooking; this was right after cutting a shitton of chili peppers.

No amount of water will help you. :(

Got to go with the groin kick. Not kicked by a human though, kicked by a cow! My various other injuries pale in comparison.

well, I've only experienced brain freeze and ball kicks, and I'm going to say brain freeze. Ball kicks don't really seem to hurt, just feel really bad...

I'd describe it as having a frozen goddamn brain.
You must not have been kicked very hard then.

OT: It's hard for me to decide considering I have not felt most of the options stated and I tend to be careful so I don't get injured too much. I do remember that getting hit in the groin hurts like a *****, so I would have to vote for that.
Well, it feels like a different kind of pain then...

Well my father has a condition called Trigeminal Neuralgia.
Dubbed as the worst pain known to man, or the suicide disease.

http://www.medfaxxinc.com/index.php?/Articles/the-suicide-disease-trigeminal-neuralgia-or-tic-douloureux.html

Hearing his screams...yeaaaaah, I think that confirms it all, really.

They couldn't show all of the opponents anatomy cause the game would get a AO rating if it did. Use your imagination.

My imagination died with my childhood...kinda sucks really. As far as the showing bits and pieces that illustrates yet again the duality of western censorship lol.

Touché. Then again, it was more painful to actually play Shaolin Monks...
Lol, yeah I know.
Especially in VS mode when you do a Fatality on someone and then the winner says "You have no hope of winning!"
