I've received a few warnings. At least one of which I will argue over until the day I die. With the new forum health system they probably won't ever go away though as I seem to get more then 1 every 6 months so I'm perpetually in the red.

Clean so far, I hope to keep it that way, since I did gett my share of warnings (and a ban) on other forums.
I got warning ages ago because of a low content post. The one I have that shows up is because I outright called someone a troll. Forgot about that rule being introduced.
I got one warning for inadequately describing my circumcision. I exaggerated the experience as
"an old pervert with a knife to my dick"
One for low content. I answered someone's question, it only needed one word to answer so COME AT ME MODS!
I have had 2 or 3 warnings at most in the past. at this current time I am clean and I plan to keep it that way.
I had one at one point, while discussing piracy in a piracy thread. I just notched it up to the Moderator being silly, but eh, what are ya gonna do.

Granted, I don't post very often, and when I do, I try to avoid offending anybody. I also tend to avoid topics that look like they have the potential to become shit-storms, so to speak.

I've never had any warnings, but I'm not Kuan Yin, and don't even know who or what that is, so I didn't vote.
I've gotten two or three warnings, though never close enough to show up as three warnings at the same time. I was also on probation once during the old system, which was weird because I never actually got a warning before, it just went straight to probation. The rest of the time, it was for being rude, which basically translates to I was having a shitty day and someone pissed me off, which I actually told them. The second time [http://www.escapistmagazine.com/forums/read/528.392362-Obama-dismisses-Objectivism-as-Immature-and-a-Narrow-Vision?page=2#15822316] was unwarranted in my opinion, but perhaps I was too hostile in my tone, I had just reached a boiling point with the user I was addressing. The third time [http://www.escapistmagazine.com/forums/read/528.385588-Ohio-GOP-County-Chairman-We-shouldnt-accommodate-black-voters#15336757] was a similar situation in that I had reached a point where it was a straw that broke the camels back situation.
I stand by everything I said, and I feel that my punishments were worth it. I would also like to say that I don't begrudge the Mods for the punitive measures (except the probation thing, but that was more because I didn't actually get my 1 million warnings that you got back then before being put on probation), and understand that the rules must be enforced for everyone.

I've had an account here for nearly a year (lurked for quite some time before that) and have never once incurred mod wrath. I feel slightly unworthy of my Neo badge, however, since I have acquired this rather unwieldy postcount through usergroup posts, rather than actual forum contribution. No prizes for guessing which group, though.
Since it isn't hard to just, ya know, be nice and polite to people; I don't treat having a clean record as any particularly big thing. If I somehow make it something silly like five years without any sort of mod wrath, then perhaps that might be worth me getting on a pedestal about. :3
