I used to do Ash Ketchum but I grew out of that pretty quickly.
I now use an alternate for my first name - as I'm a Frederick, I can shorten it to Fred or Freddie; Rick or Ricky. I like to throw them around a bit.
Usually just add a relatively common surname on the end and I'm gold.
It's either that or George Fredericks. Staying close to my name is an easy way to remember what I've put down, I guess.
Occassionally I call myself John Doe. Cookie for anyone who gets it.

I don't have one (usually if they want a name on an online form and I don't want to give mine, I just write "none of" and "your business" for the first and last name slots).

My favorite name to use on websites where i want a legit name, and if i used a fake name ever i would use Justus Cross. The first names the same as mine but the last name edition makes it way more awesome. But yeah "Justus Cross".
