Film: The Green Mile. That ending in the past had me in manly tears all the way through.
TV: Outrageous Fortune (A New Zealand TV series, it's amazing), Aurora's death and the subsequent episode of Van going insane.
Book: Storm of Sword by George RR Martin. namely the Red Wedding.
Song: ....I don't listen to depressing music. But I guess out of what I've got, I'd say LP's Crawling.

Don't know if it's been mentioned, but one of the saddest songs out there is "Dance With the Devil" by Immortal Technique. Just a fair warning if you choose to look it up, it is some seriously messed up stuff. Be prepared.

The end of Toy Story 3, the panning camera shot from the table over to Tiny Tim's empty chair in The Muppet's Christmas Carol, The Luck of the Fryrish/Jurassic Bark.
Also, when I was a little kid, Pikachu's crying scene in the first Pokemon movie really hit pretty hard.
Grave of the fireflies, IDK how anyone that's seen it can say otherwise. Sure luck of the fryish and jurassic bark are sad (Very sad) but grave of the fireflies dominates.
I'm pretty sure it got an R rating in some countries because it can be traumatizing for children.
Everyone should watch it at some point.

The main protagonist performs an abortion on herself with a needle off of an African thorn bush. What makes it worse is that her actions are an attempt to save her potential future as a nurse, except
she is shunned by her husband and society for her actions and ends up pregnant again anyway, choosing the second time to burn herself alive.

The saddest film I've ever watched is Memories of Matsuko, that film made me sit in shock for a good 10 minutes after it had ended. It made me reflect on life more than any other. It's a fantastic film.
The saddest game I've ever played is Silent Hill 2, that game has brought me to tears more than any other.
I can't think of a piece of literature that has made me cry the most. I've read so many, but I can't seem to settle on one sad one.

Not sure about the saddest movie, but the end of "Dr. Horrible's Sing-Along Blog" left me feeling empty inside.

When Carlos died in Saints Row 2, he was just a kid who saw you and the saints as a sort of idol so he breaks you out of prison so you can redirect the saints without a clue that he isn't prepared for what he's going to see and do. After you criticize him for not getting enough information o. The gang he's supposed to be watching you have him help you to harm the leader of the gang he's watching. After the leader finds out who did it he had Carlos tied to the back of a truck and dragged around the streets until you stop the truck, but once you get there you see that Carlos can't be saved so you put him out of his misery realizing you were the one who caused this.
Book would probably be Flowers for Algernon.
TV show would probably be either the final scene from Blackadder goes forth or some of the moments in Scrubs.
Can't think of anything in a game that's particularly moved me, possibly the final conversation with Garrus in ME3. For some reason when he said to meet him at the bar in heaven...
