To Zanarkand for me was definitely the saddest song EVER in my long life of gaming, and it really set the tone for the whole game. Which was depressing.
It is a wonderful song, and FF series is full of truly sad songs. Uematsu knows his stuff. I find Sorrow and Regret from Golden Sun to be really heart-breaking. There are just too many sad song out there to be listed in here, but currently this one impresses me the most.

The saddest I've ever heard is the 'you have died' result screen for the final mission in Resident Evil Outbreak (the single piano melody made me cry dammit!).

The one that springs immediately to mind is Old Snake from MGS4.

The vast majority of the Silent Hill 2 soundtrack is pretty melancholy as well. Apart from Angel's Thanatos. I've played through 21 times and never noticed it in game. Does anybody know if it's there at all? And if so, where?

The music to the credits for Twilight Princess (especially when it slows down) almost had me in tears. Of course, the cutscene that plays and the fact that I never wanted the game to end may also have been a factor.

That depends, can I count Beethoven's Moonlight Sonata from Resident Evil 1? If so that, without question.

If not, probably some of the piano tracks from the last few cut scenes of FFX.

I believe it's 'Scars of Time' from Chrono Cross. That tune was just...

Either that, or 'To Zanarkand' from FFX, man that tune was depressing.

It is just awesome, hearing it, after you got through the game on "Dead on arrivel"- mode when Mona survives the end, Max's problems are solved and he says: "I had a dream of my wife... She was dead, but it was all right" and this song begins to play... awesome.
