Not sure about "ever", but saddest song I can think of off the top of my head is Johnny Cash's cover of Trent Reznor's Hurt [http://www.youtube.com/watch?v=clq01TXQR0s]. So good that Reznor declared that the song was no longer his after he heard Cash's version.

Runner-up (again off the top of my head) would probably be If You Tolerate This Your Children Will Be Next [http://www.youtube.com/watch?v=MWu-QOVekeU] by Manic Street Preachers, which is disturbing and surreal as well as somehowextremely sad.
