When I was small, this is what had me on the edge. I played through only to be greeted by the most creepiest boss I've seen in my short existence. Andross, an old floating head of a primate. He tried to eat me. Asshole. Didn't help that he turned into a brain during the second half.

Suffice to say, I lost the first time I fought him.

It scares me because someone actually thought there was a good idea for a video game in that...
If you'd like a serious answer though, I'm not really sure I can provide one. I haven't had much experience with 'scary' games as they never really intrigue me. Maybe a few parts of Quake 4.
I'm going to be playing Alan Wake soon and I've heard that game could fall into the category, so we'll see.
I will say a few parts in Heavy Rain were quite fear-inducing.
It scares me because someone actually thought there was a good idea for a video game in that...

WTF nintendo. I know you haven't released a single game without the whole "OH SHI-" thing, but majora's mask just said "Oh who are we kidding" and had some of the scariest moments ever. and one of the you go through repeatedly, in most/all timelines: the deku scrub change.
"NNGG...AAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAAARRRRRRRGGHHH" while link(kid link, in fact), in his shapeshifting agony, stares into your very soul. I find it rather unpleasant.
not to mention how human some of the characters felt in comparison with... most games. ever.
doesn't make you feel all that good when you leave them to die in an alternate world. repeatedly. in order to win.
seriously, nintendo should make a stright horror game. and not tell us its a horror game.

Fallout 3 with the (I think that's the name) Ambient Terror Music radio station mod.
I thought "hey, I can smash my way through Deathclaws, who's scared of a little music?".
So I went into one of the metros, tune in to the station, and nearly shit my pants. There were just a few ghouls around... and a few dogs... and I was sneaking like a scared little biotch :/
