The last level in Condemned: Criminal Origins.
Your trapped in a small house with a murderer...still scares me.
^This. Terrifying to the highest degree.

For some reason, the last level of Time Commando scared the hell out of me, but that was mainly because you were floating in zero-space fighting a giant shark with spiky balls of energy.

God damn you shark. I could never beat you. Damn you to your digital hell.

I swear the scariest thing I've seen in a game is the final boss of Earthbound, Giygas. Just look up Youtube videos of the final fight and you might see what I mean.

The Secret Cow Level.
May I ask WTF that is? :3

EDIT: I really shouldn't be on this thread since it's 3 in the morning...
A Secret level in Diablo 2 where all the enemies were cows with halberds. it wasn't scary at all and was meant to be funny.
Idk, the sounds of "Mooooo..." haunt me sometimes in the night. XD

Any level from Penumbra: Black Plague. You... have... no... flipping... weapons. There... are... almost... no... flipping... enemies. You run away on the rare occasion you see one. You run very fast.

And if I had to choose, I would pick getting chased by the infected doctor in the computer central. He tells you that he needs a saw to cut off his hand to let you enter another room. You give him a saw through the door. So he does cut off his hand. AND THEN CHASES YOU AROUND THE LEVEL. You actually have to hide in a server room, bait him in, and lock him in. Plus, your character is actually human, and has panic attacks when looking at enemies.

The department store level in Condemned. I haven't really played that many horror games, but that's the scariest I got.

Also I don't get why so many people say Ravenholm was scary. Yeah it a little creepy, but not scary.
I agree I thought the preacher dude was more badass than scary.
He's the reason it wasn't scary or even a little creepy.

I still think the school level in Condemned: Criminal Origins was scary. Or the store level, where you couldn't tell which mannequins were enemies until they had already started attacking you.
This. The part where ever time the lights come back on they'ved closed in on you further scared the fook out of me...

However it was nothing compared to the almighty LOCKER SCARE.

Anyone remember the part in Broken Steel where the deathclaw drops a corpse from the floor above you and lets out a growl?

That part is intense if you're playing on headphones, made me jump the first time.

I always thought the nightmare scenes in max payne with the baby crying and blood trail real creepy.
Thats just disturbing... not really frightening.

AS far as things that have made me get up and leave the game/computer.
WAY back when survival horror was just getting its foothold in the gaming industry... Resident Evil 1 and 2 were out. Silent Hill was JUST around the corner... and since everyone know Dinosaurs are fucking scary... Capcom decides to release Dino Crisis.

Overall... the game itself isnt scary and Im sure if I were to go back and play it... I wouldnt think it was frightening... but I was 12... Surv.Horror was relatively new. And the scene when you get into the Administrators Office... and OUT OF FUCKING NOWHERE... BAM T-rex breaks his face into the office plateglass window. In retrospect... I have since learned, that "Large plate glass windows that arent broken are about to be in an attempt to make me shit my pants" http://www.youtube.com/watch?v=Dp-59rTKNi4

That has the largest "holy shit" moment for me.
Another close one, was in Silent Hill3, when you are going through the amusement park, and you are in the Borely Haunted Mansion (Skipto 3:25) http://www.youtube.com/watch?v=dPOpkif0SDk&feature=related

Now, if inducing a heart attack is the same effect. F.E.A.R. did a decent job of making me waste ammo on absolutely nothing. F.E.A.R.2:project Origin upgraded that when the phantoms WERE LIKE WHISPS OF AIR AND LOOKED PISSED!.
http://www.youtube.com/watch?v=_Vib42kWkd8 (phantoms from FEAR2)

I have not played through Condemned: Criminal Origins but I hear thats one hell of a horror game. Ive got it in my GameFolder just should install it and play it.

And... as far as just the most unerring scene ever. When you get to the first set of apartments in Silent Hill2. You come to a barred hallway. And farther down the hallway, you can see something. Its just standing there, swaying back and forth. And later on you have to go back up to it... and its gone. It hit me later... PYRAMID HEAD WAS RIGHT THE FUCK THERE!

Im new be nice to me.
No
Ignore him, he has an ailment that makes him somewhat of an asshat at times.

(By "at times", I mean all the time)

More on topic:

I'm straining to think of a game that really scared me at all, but I'd have to go with one of the many times I've turned around a spotted that a Deathclaw has spotted me in Fallout 3. Especially because I play on Very Hard.

Shalebridge Cradle(it used to be an orphanage and then an insane asylum, for a period of time both.). Thief 3. I had to have someone in the room with me while I did that level.

I actually never finished Thief 3 because of the Cradle level.. *lowers head in shame*

I watched the Cthulu level video from page 4 I think, and holy fk, that was intense! I also just looked up some Condemned videos after watching the mr Tibby (?) one on page 4 as well. I didn't find any of it that scary unfortunately, as I quite wanted to. Maybe I just wasn't looking at the right ones...? :s

A lot of people mention the haunted hotel in VTMB, I never felt that scared, you know because I was a VAMPIRE.

The Shalebridge cradle (and a weird error that messed up all the writing in my objectives menu) is the reason I never finished Thief 3, now I think I'll give it another go now.

Condemned, I found the school excessive after the well crafted atmosphere of the library.

In condemned 2, I never found anything that scary, because the tables had turned, I was the crazy homeless man in the shadows that other should fear.
