In nazi zombie map on COD:WaW when your firing out one window only to turn around and be downed by a sneak attack, scares me every time.

Honestly, no game moment has freaked me out so much as the opening screen to this Pac-Man rip-off I had when I was eight (anyone remember Pac-Guy?). Heavy metal blared in the background with the little pixelly enemies swarming all over a realistically rendered skeleton. At some point I stopped being scared and started thinking it was awesome.

Every encounter with William Birkin in RE2. Every time your like, oh what can he possibly do this time that is not as fucked up as last time? That is when he pops up four giant claw arms and chases you around a fucking tram. And don't remind me about the goddamn elevator. Birkin is an asshole, but definitely the scariest asshole you will ever run into. ( Mr. X is pretty fucked up at the final fight too.* Shivers*)

I was listening to an audio diary in BioShock and a little into it I decided it would be better if I was watching the entrance instead of staring at the wall and a Splicer was standing right behind me!

Didn't even attack me until I looked at him, which leads me to believe it was scripted but I've only played that game once so I'm not sure.
Was it in the room that frosts over when you enter it? Happened to me too, but on the SECOND time I played. The first time I must have some how missed him which kinda is creeping me out NOW thinking about it. Alma does that alot too in FEAR 2 and has gotten me multiple times.

That reminds me of something similar that happened to me. Bioshock never really scared me except for this bit. I entered a dark hallway, burned a splicer and turned to a Gaderer's Garden to buy something. I felt a little eerie so I turned around. There he was, the burned splicer I had just tried to kill second ago. He was just staring at me all burned up. His disfigured face was taking up half the screen too...

Getting caught by a Nightmare in Psychonauts made me afraid to exit the houses. I'll stay and bake pies with the G-Ma- er, nice apron wife lady, thank you.

And going around the Realm of Torment in Guild Wars always makes me a little freaked out. I was fine with fighting undead and the apparent cousins of the Fludd in the first two games, but then they stick me in some place with...teeth in the ground...and caves made of flesh...and those damn nuking Dryders. Yech.

A scary moment was playing 5 Days a Sacrifice
Uh, it's 5 Days a Stranger or 6 Days a Sacrifice.

But really, both have rather startling dream sequences.

I played (and finished) 6 Days for the first time a while ago. I'm still feeling rather creeped out.

Playing Morrowind on xbox for about 3 hours and then it freezed. I realized i hadn't saved since the beginning. So I had to do everything again. Such a time sink.
I reflexively reach for the quicksave button after just about every battle because Morrowind is horribly buggy on the PC and can run for hours or seconds...you never know when the next CTD is coming.

My first real fight with a Big Daddy in Bioshock. I thought they were lumbering hulks, just stay on the opposite side of the room and fire away and HOLY CRAP HE JUST CHARGED AT ME LIKE A GAWDDAMN TRAIN! RUN! Dammit, why won't I RUN! He's got me off-balance, crap! Reload Faster, Dammit!

Even knowing how each type works, they still make my palms sweat.

The Darkness.

I was feeling suicidal and walked into the subway tunnel, waiting to get run over. A while passed and nothing happened. I turned around to go back, and was face-to-face with a speeding death cart.

It wouldn't have been as scary if it weren't for the fact that the developers actually make Jackie (the player character) gasp right before he gets hit.

The haunted beach house in Vampire The Masqurade: Bloodlines, Although at the moment I'm playing through Call of Cthulu so we'll see if anything in that game can top the beach house.

I found it was scary (scariest has probably already been mentioned) in Oblivion when I first went through the gate. Scared the willies out of me.

Been there. I eventually quit Morrowind because it kept freezing at the same point. It was terrible! D=
