I'm gonna be a maverick here and pick one no one else has...
It was in Fallout 3 and I was in Vault 87. This must have been my fifth or so playthrough so I knew what to expect. Anyway, I saw a radroach coming towards me and didn't wanna waste ammo on something that literally couldn't hurt me. So I shot at it with the assault rifle, one bullet at a time, walking backwards.
Suddenly, my guy stopped moving backwards, as if he had hit a wall. But there shouldn't have been a wall there. Just as I'm spinning around FOUNDYOUARRRGGGBARGL!!!!! I had backed into a supermutant and he yelled just before I could see him.
I scream, jumped, and tossed my controller in the air.
Another case of REAL FEAR not just jumpy fear was in Fallout New Vegas. I was doing that Dead Money DLC and was at a part where you fight ten or so of those ghost-y guy things. I had barely any ammo left, but definitely enough to take them.
Anyway, I could hear them and they were on my radar, but I couldn't find them. Every once in a while I'd stumble on one or two and fight them only to get my ass handed to me and run away into more or to see more in the distance.
There was no way out and nowhere to hide so I literally hid in a corner. I started freaking out in real life and felt like I was on the verge of tears. I couldn't handle it anymore, and I have no idea why. I felt that if I saw one of them I would just lose a shit so I got up and shut off my Xbox.

Also, deathclaws never scared me. By the time I actually would meet them (Level 9 or 10) I could easily take them. I always found them cooler than they were scary.
