That sequence from Tomb Raider: Anniversary was the only one that made me feel scared.

And people often quote the infamous "Mirror Room" in Silent Hill 3 as the most terrifying/squickiest moment.

Anything in Doom 3 scares the hell out of me...to this day I have yet to finish it

Yet I can play Quake 4 and not even give a rip

The scariest monster? I've got one: it has been at war with its own kind since the dawn of its creation, struggling to subdue all that crosses its paths, burning, pillaging, murdering, raping, enslaving, etc, all that oppose its might. It will find you, and it will kill you for some contrived reason. Only the reason is a lie; it kills for the sheer animalistic pleasure of it. I'm talking about:
MAN!

Ok, ok, something real. Let's see...Pyramid Head's been taken, so I'll go with those kamikaze freaks from Serious Sam. That scream...it's the harbinger of destruction.

I'll agree with the Regenerators/Iron Maidens from 4. Basically anything like that where you hear it before you see it will always provoke a tingle of fear to turn into a steady stream. However the chainsaw guys from 4 and 5...formidable yeah don't get me wrong but they don't get to me really. I'll never look at a chainsaw again in the same way but nothing beyond that heh.

Yeah I've still to see any Texas Chain Saw Massacre movies..

The horror I personally put forward are those poison headcrabs from Half Life 2. Doesn't help that I'm a victim of arachnophobia but these things just freak me out so much. Their squeaking, crawling and leaping in the face with the toxins...I can't even play any of the levels that they inhabit which number a few as some of you know, especially episodes 1 and 2.

This. I'm pretty sure I blocked out the entirety of the "Bottom of the Well" level when I played it as a kid, 'cause I recently went back and played it. I pretty much went "What the fuck is this? I remember there being a level under the well, but was it really this shit? There's blood and gore and crazy arm monster things everywhere!"

I'm surprised I wasn't traumatized. Or maybe I was....

Have to agree with that one as I needed more firepower than I currently owned about Freeman's person.
It was on on the highway mission where Gordon has to navigate under the railway bridge. Some of the normal zombies lurched from hiding but that was ok. No real problem there.
Got into the control box, push the required switch and shot down the helicopter thereafter. Went back through the various storerooms and lo and behold, one of the poison zombies came shuffling waywardly down the embankment

"Just look at the face: it's vacant, with a hint of sadness. Like a drunk who's lost a bet"

Brought the RPG to bear, aimed into it's path and fired. Hit him square on and he just pinwheeled below into the water. Watch as my face turns from accomplishment to horror as one of the lil' nippers fell or jumped off, still on the embankment mind and scuttled out of sight towards the rooms up ahead.

The rest of that I can't remember, either out of memory-blocked fear or just old age

I've got a half one. Really the only "monster" to scare me in a game.

And it wasn't even a fucking monster!

I was playing Ocarina of Time, and was in the water temple. I was borrowing a friend's guidebook (because it was the fucking water temple), and the jerk thought it would be funny to mess with me, so he scanned a page and replaced the text and pictures (believably for the time, fifth grade I do believe). It said that the dragon pipe that holds a switch would come alive after I used the iron boots to sink to the bottom. I hated battle under water, so naturally, I did not want to do this fight. And it freaked me out, to the point that I stopped playing for a week. When I finally built up the confidence, nothing happened, and I bitched out my friend the next day, who just couldn't stop laughing.

Another one would be Nemesis, though that was mostly due to how intimidating it was, not how scary it was. And the mannequins that follow you in the original Condemned would have gotten me had I not been expecting it. But since we're on Condemned...

Pretty much anything in STALKER at night if you have the Oblivion Lost mod where you can only see what your flashlight is looking at.

System Shock 2 had some good ones between invisible giant spiders, zombies that apologized as they tried to brain you with lead pipes, and the nurses....

Oh and the facehugger from AvP '99 skitter skitter skitter pause....facerape scream. Got me so paranoid that I shot all my grenades at the floor whenever I heard the skitter sound.
