The bloody hand thing in the shadow temple and in the well from legend of Zelda Ocarina of time. Used to wet myself playing that game as a kid. Not so much has changed these days.

As the post above says, Skedar, ninja's in FEAR weren't that bad, had an itchy trigger finger and full penetrator ammo (think big nail-gun with perfect accuracy) and ALMA was more psych fear that i found predictable (except for office cubicle surprise).
Pyramid Head was scary back when i first played SH2 when it came out. But my biggest fears were the re-deads from OoT, was so paranoid with them i played the day song to freeze them every 10 secs when i knew they were around... now i just laugh as i biggorron sword them to death.

Most recent scare to death was the spirits in FEAR 1/2, fear 1 because i was paranoid then, fear 2 because of a hallway were the only way to see was with the muzzle flash..... didn't help i was having fun spraying ammo on enemies right before the area.

honerable mention to the head divider in Dead space... it steals your body when it kills you (see link--> http://www.youtube.com/watch?v=xAOKEIJ74R0&feature=related )

Don't know if anyone here has played the game Vampire The Masquerade: Bloodlines but the Tzimisce creations you encounter in the sewers scared me the first time through the game; just because they lunge at you and they are small and hard to hit.
You mean those creepy little bastards with only two legs that run incredibly fast and make that horrible dry hump noise when they attack? Bloody hated those things.
Thos things were especially fun to deal with when you play the game at 1am in the morning. In retrospect they weren't scary, just.... shocking if anything

Heaven Smiles from K7

How about that entire game!

I hated those creepy things. I got banned from playing it with my mates. Turn a corner and BANG! 5 of them staring at you.

Glad I never have to play that again

Yes those things. Wouldn't even call those legs i like to think they're just arms with a head in between. They strangely remind me of the facebugs in Demon's Souls.

I have been playing Amnesia:Dark Descent and so far every single monster has scared the living F*** out of me. Its not because of how they look but more the way they moan and scream and the fact that you cant fight back, only run

Dianoga, particularly the giant one that fills an entire room.

Other beasties from Star Wars such as Kell Dragons or Noghri too, but Dianoga wins for size.

The namesake deity from Call of Cthulhu: Dark Corners of the Earth. He never even appears fully on screen. He just pulls people down manholes. Scary as hell.
But Cthulhu's only appearances in DCotE are as statues that drive you insane if you look at them too long, and he wouldn't be able to fit into a sewer system to begin with... In fact I can't seem to recall seeing anyone being pulled down any manholes in that game, in which level does that occur?

Anyways, on-topic, I can't recall any videogame monsters that scared me out of playing video games like, ever. I've never extensively played any of the Silent Hill games, so that could have something to do with it.

EDIT:

How the hell did a topic from September appear in the 'hot threads' tab in July? Maybe I should look at post dates from now on...

There shall only be one TRUE answer. Ladies and Gentlemen: PYRAMID HEAD!

He's big, he has a giant sword, his hobbies include murder, rape, and beethoven, and HIS HEAD IS SHAPED LIKE A FUCKING PYRAMID!!!
True-er words have never been spoken, but those f**k ugly gatherers from Amnesia are kinda scary too.
