In Ocarina of Time I went through the very first dungeon and faced Gohma (Gomah?). Anyways, during the part where you have to shoot her down from the ceiling I couldn't find her, so I started to pan the camera around (in 1st person mode) when she drops down right beind me. Her eye was, like, two inches away and filled the entire screen.

Scary as hell.

I found the near-ending of portal (no spoilers here) to be rather suspensful, and I was in suspense the whole time until the climactic battle, which was just awesome.

In Portal, when I entered the first decrepit "Cake is a lie" room I had the terrible notion that I was about to discover some horrible mutilated previous test subject. I didn't stay in that room any longer then necessary to glimpse the wall and flee and in terror. From then on, I was really scared to ever enter one of those rooms again.

The fact that Kratos wears a skirt. I know we are not talking about characters but Kratos with out a skirt would put me at peace. But to appease the thread, the Little Sisters after you return them to normal. They always wave. ALWAYS.

Cthulhu: Dark Corners of the Earth is an excellent adventure, but I would've preferred a bit of toned-down madness.

Something just happened to me today actually, and I friggin' hated it.

I dusted off Ehrgeiz for the PS1 today and playing through the action-RPG half of the game pissed me off because I'd JUST be leaving a room and suddenly a bunch of shit would spawn into the room with bright lights and loud noises. I jumped EVERYTIME... I mean I knew it happened but it was all so random, I never knew when exactly it would.
Ugh, Ehrgeiz... I only have one memory of that game.

Giant octopus.

When I was younger, I used to get really scared at games, were there was empty suits of armour that came alive...

You cannot grasp the true nature of Giygas' attack!
The thought of that is rather unsettling, to say the least. I kinda wish I hadn't had that fight's ending spoiled before I got around to playing, though. It would've been EPIC instead of just being awesome.

Crysis.

I accidently drove a jeep off into the ocean and looked down and nearly shit myself.

I cannot !@#$ing STAND not being able to see the bottom of the water, and the black void I saw then nearly stopped my heart. Needless to say I got out of there pretty damn fast.

I used to be scared of Gohma, too ( I was six/seven, I think...). When I beat her though, I was so proud...I ran down into the basement beaming, telling my Mom about it.

Try terrifying more than epic.
It gets even worse when you find out where the inspiration for this battle came from.

Anytime I fight an enemy that you can't kill, permanently stun or incapacitate.

Particularly if they act dead, but get back up.

I jumped a mile once when a Random Encounter appeared in FFIX... I felt very silly.
