as a kid the screams that the re-dead did when you got to close scared the piss out of me in Legend of Zelda: Ocarina of Time

Being 11 years old didn't help, but yeah, the re-dead are on my "scariest monsters of all time" list for sure. Some parts of Bioshock can also be pretty disturbing - I generally want to explore every corner of a level in games and I kept anticipating some horribly mutilated corpse or splicer ambush around the corner.
