I'd have to say that gameplay wise, sequels normally do much better than the originals. Stories normally suffer because it is no longer original and it has to work from the previous title. The other reason the story is often worse is due to the fact that it has to work off a story that (normally) has been completely wrapped up.

I would make a list but it'd probably be easier to list the games that I think the sequel didn't improve on the original.

I terms of movies about the only ones I can think of are the Dark Knight and Toy Story 2.
Aliens, Terminator 2: Judgment Day, Evil Dead 2: Dead by Dawn, Road Warrior, Empire Strikes Back, X-Men 2, and so many more. It does happen every once in a while
Aliens is better than Alien?! Are you high?

But yes it happens constantly with intended sequels (movies that are set up as part of a trilogy ect) but most of the time with movies that don't lead them selves onto a sequel easily the movie suffers, even if it is good. Obviously most of the ones you listed are exceptions.

I think these stand out:
Thief II
Hitman: Blood Money

Then there's the games which are too obvious to list. You know, Super Mario Galaxy vs Donkey Kong. Errr, yeah.

Saints Row 2 is most definitely better than the first one. But for the life of me, I can't remember what the first one was called.... Hmmmmm.
Sinner's Column I

A lot of game sequels are better than the originals, but the best example is Ratchet & Clank transforming into Ratchet & Clank: Going Commando. Which is fucking awesome. The original was fun, but the aiming sucked. The additions of the later games far outclass it.

kingdom hearts is much better than number one, so much in fact that i can no longer enjoy the first, trouble is kingdom hearts 3 looks amasing and now i am worried i would no longer enjoy number 2

I think it's safe to say Resident Evil 4 was vastly superior to any of the others in the series. The controls were drastically changed for the better.

ofcourse there are sequels better then the oridginals, like kingdom hearts 2, its so much more fun then KH1
I miss the diffculty of KH1.

Anyway, Silent Hill 2, Street Fighter 2, there is indeed a lot of good sequels. Most of them are just overhyped or lackluster.

I think that sequels can perfect ideas from the original, but they usually forget the spirit of the original after one or two iterations.

I'll take everyone's word for most of these (haven't played them). I guess I'll just add Tekken 5 to the list.

I'm torn on some things, though. Baten Kaitos Origins had much better voice acting (like that was hard), a more fast-paced battle system, and much more convenient navigation, but the story was about equal, and the battles were more subjectively than objectively better. The Tales of Symphonia sequel has, I'd argue, a more fun battle system, though it's not quite up to the other standards (so far, I'm only halfway in at most).

Its not so uncommon. I doubt many consider Arena the best TES game, though alot of what is core to TES has not yet been developed then. (skill based leveling)
Zelda and Final Fantasy also from what I have seen a sequal is considered better than the origional.

Devil May Cry 3. I don't even know how they managed to screw up 4 so bad after finding such a perfect formula for a DMC game.
