I ended up getting shot by an arrow trap that I didn't see midjump, which launched me in front of another trap. It shot me so that I would get impaled by spikes. Honestly, I was already dead from the second arrow, but even if I lived, the spikes would have killed me anyway.

I got blown up for the first time by a few Creepers in Minecraft.
Thank God it was just a game world I created for dicking around, trying out some new mining strategies, I was gonna delete it immediately after leaving the game.

I was playing single player CoD Blops, because playing multiplayer doesn't count I assume and I forget. I was fighting some enemy of America or another, and had the choice to throw back a grenade, so I started throwing it back and it blew up in my hand.
Then the nerve killing part was when the game happily told me "Look for the grenade indicator to know when grenades are near" I almost threw my controller at it, so I respawned almost right before the grenade was thrown, and just took cover behind a wall and laughed at the enemy not knowing Mason can see the future.

Mauled by an absolutely massive horde of zombies on "Five", wave 18. My friends didn't last much longer.
A few minutes ago I tried to be a Sorceror. I failed horribly and died a minute after stepping into The Cathedral...
I can't recall if I got the "Game Over" screen, but I was put in an unwinnable situation while being attacked by a Desire Demon and her flunkies. Damn post-cutscene fights, getting the drop on me.
Normally in games I'm a nice guy. This playthrough I was trying to be pointlessly evil.
I slaughtered the entire population of Megaton excluding the kids and butchered them until I could pick up their heads and drop them in the bomb pool in the middle for giggles. Then I killed Jericho my trusty assistant and threw his head onto the gory pile as well. I wasted all my ammo shooting it off either at the pile of bodies or the cold uncaring moon then used the last of my grenades to blow myself into oblivion... Yeah...

Mass Effect 3 trailer got me playing a bit of Mass Effect 2 again- playing as Vanguard.
Going through Jacob's loyalty mission- forgot about the Heavy Mech that's there near the end- used Biotic Charge on one of the crazed guys, left myself out in the open and without cover, Mech snuck up on me from behind the wall. Got mowed down pretty quickly before I could reach safety

I am currently playing Pokemon Crystal to relive me Gold nostalgia. Got up to Falkner and my team was slaughtered. My Spinarak, Zubat, Hoot Hoot, and Poliwag were all defeated. My Onix took out his Pidgey but then his Pidgeotto took out both my Onix and Cyndaquil with that damn Mud Slap.

Red Dead Redemption:Undead Nightmare I was trying to save a town, instead the zombies pulled a group attack of doom and I got eaten while slowly running out of Ammo. It was actually the only time I was actually excited to die.
Went upstairs to get sustenance whilst simultaneously in Mortal Combat with a Deathclaw!

Dragon Age. My Warden got repeatedly punched in the gut by an Ogre and died when Alistair decided he didn't feel like saving her with Shield Bash. I know it doesn't always work but he could've at least tried after she'd screamed at him for help with blood frothing out of her mouth...
Turns out sometimes when I select a skill for them to use they can pretty much just turn to the screen and say no.

Oblivion, put the difficulty up to maximum and try doing the quest on the shivering isles where the two groups of guards fight each other. I hit (by accident) one of my new allies with an AOE spell and hurt several people, damn, everyone turned on me and I was owned by so many NPCs
