OT: Well my main character died in FFTA2, but it was against Klesta, on hard, and I was level 10, it was level 14 (I won because there is an invincible spot on her side where she can't use peck or devour, and she can't use territorial marking because it would hit her too, so she sat there attacking the unkillable but non-fighting NPCs while my last guy sat in her blind spot gradually beating her down, 27 damage at a time). But yeah, my gladiator won the match, but not before my Main character got squashed by a giant cockatrice.

EDIT: Yeah, stupid grab attacks are the reason I now only play as archers, bow-mages[footnote]Which actually work fairly well, you get the magic you want for the glyph of repulsion and the stinging swarm line (also grabbing arcane shield) and then pump dexterity and nothing else. Then, when you get arcane warrior, you get attack boosts, wear whatever armour you want, cats repulsion glyph if you get melee'd, warding glyph if you are being attacked by archers or mages and keep arcane shield on at all times (if you have spell wisp on first you can boost the bonuses from all the buffs you'll use as an arcane warrior)[/footnote], or as strength shape-shifters [footnote]which actually pwns the crap out of stuff if you get the glyphs and the earth line (for earthquake) and have wynne set to constantly cast regenerate on anyone with less than 90% health[/footnote]

Trying (unsuccessfully) to farm a way too high leveled mob in FFXII. God I hate that game.. why must I try it again u_u

Previous to that, In Ganon's Tower in A Link to the Past.
Yup I used my snes and ps2 this week while my wii, ds and ps3 gather dust in the corner.

Fallout new vegas

getting poisoned with 1hp and no stimpacks and then going into a building so it autosaves.
the result: every time the loading screen finished my body would crumble to the floor and would take me back to my autosave,which was me dieing. I made my own paradox :D

Close. It's the scene in the Circle Tower with the Charmed Templar. Not that hard of an encounter really, really, but she got the drop on me after the cutscene ended and snap freezed us which got my mages killed. :/

That's my only real problem with DA:O. Half the fights feel more like you win or lose by random chance and luck more than any actual skill.

The one I especially hate is the one of the second(?) floor, where there is a room of like, 6 templars, so my shape-shifter (who uses glyphs) was basically a 'sort of' warrior, wynne basically sat there spamming regenerate, alistair got beat down by all 6 templars and lelianna only had one helpful ability (scattershot). So yeah, that battle took some careful paralysis explosioning, earthquakes and ganging up.

I got PWND repeatedly at Deathmatch multi in Unreal Tournament 2004. My score at each round was somewhere around 8 kills to 30 deaths.
So I died.
ALOT.

Another one for Super Meat Boy. World 2-2, I think I've died about 20 times before realizing that I cannot just free fall into the bandage and directly to Bandage Girl. The other 200 or so times have been from leaning slightly too far to the left or right while in midair and spiking myself either on the nearby ledge-spikes or the center pillar of spikes.

Just died on D&D Online because my healer hireling decided it'd be a good thing to run through a large sonic blast trap....

Ran out of healing pots on the last boss! Augh.

Team fortress 2

There I was, the prime of my life, a scout with a two kill streak, before the following story:

I Grabbed the Enemey Intellegence, after darting across the roof of 2fort, and daringly jumping into thier base, keeping the sniper occupied with a burst from my scattergun. I recieved a small burst from his sub-machine gun, but it was not enough to deter me. Dashing into the basement, searching for the Intel, I found a room with only one other exit, but more importantly, the Intel.

With a burst of anticipation, I charged in, only to dive straight back into cover the moment I saw the sentry gun and the engineer, standing guard. This was going to require wit and courage, to pass this obsticle. I noticed a patch of ground, where I could get a clear shot with my pistol, but the sentry would not notice me. Slowly, aware that any mistake would resault in a face full of missiles and bullets, I began to unload my pistol into the sentry.

Success, however I now had to deal with an angry engineer. His shotgun shells rattled the walls around me, I needed to do somthing and fast. In a moment of both bravery and idiocy, I jumped out from my cover, suprising the Engineer, but more importantly, giving me a chance to get my shots off. Needless to say, the Engineer was no match for my agility in the field.

Alarms rang around the Enemey base. They were all coming for me, I needed to be quick. I rushed up the stairs to their spawn. A Heavy burst out of the Spawn, minigun rolling. I dashed past the advisary, dodging the bullets and dropped down to the ground level of their base.

Knowing that the heavy would soon be on my tail once more, I made a quick decision to head underground, to the sewers. With the sound of the spinning minigun barrels echoing down the stairs and tunnels, I found myself trapped between a battle for dominance in the sewers, with an enemey soldier and pyro engaging with a few members of my team.

Knowing the pyro would be the biggest threat to get past, I whipped out the sandman and the pyro got a face full of baseball. Wasting no time, I unloaded a scattergun burst into the soldier, and rushed down the tunnel towards my team members, with rockets whistling past me.

I reached the end of the Tunnel, to my team mates, but only then did I realise it was a trap. An Enemy spy had murdered my team and disabled their electronics, and greeted me with a revolver. No quick thinking could get me out of this, as the revolver rounds smashed into me and punctured my skin. My helpless ragdoll was thrown against the wall, whereupon it slumped to the floor, intel still on my back. And thats when I ragequitted.

And that is the story of my last death.

Opened a door. There was a Director behind it, eyeballing me with the many eyeballs he has especially for the job. "Okay" I said, and closed the door again very quickly, Doctor Who style. Pincer attacked him: Sent Aerie and Jaheira around to the other door then went in with Minsc, Myself, Yoshimo and Nalia in the first one.

We killed the Director easily, but the falling shards smashed me stone dead.

I'm currently doing a Handicap run in Demon's Souls.
- max level is 130
- no HUD
- Brightness = 2 (default is 6)
- going to NG++++++++
- online, no co-op.

Suffice to say, this is very difficult. Most caves are a hell to travers and you're never really sure how much health you have or stamina. Atleast in combat.
The last thing that killed me, just now, was Flamelurker. Though I kicked his butt a few tries later (barely might I add).

Hmmm.... How about the time when I lost 60% of my army in Medieval 2: Total War... I didn't lose though.....

Still I never lose that many troops in one battle.

on the run through an enemy base, had my back to the wall with dozens of guards in the corridor and pouring through the door, i was emptying round after round into the crowd until the blood started pouring down the screen... to make matters worse, it was an escort mission!
