Lost jack, you gotta lose one in the end, its innevitable, it was cool, she was all like "damn it, i didnt supose to care"

But i hated myself for bothering with the ship and my teammates, you see its some much darker and better when so many people die, its a better "Story" atleast for me.

I lost Mordin and Tali in the end and gave the most potent, destructive shit to Cerberus. Also I looked like Harbinger. Oh, and I banged Miranda in front of Tali.

Kept the genophage data.
Counter-Brainwashed the brainwashed Geth.
Chose not to advise the Quarians either way on the war.
Let Garrus kill the traitor.
Slew the Thresher Maw.
Let the savages tear Jacobs father apart.
Blew up the base.
Walked out with the whole team alive.
Had tea and scones to celebrate our spiffing victory.

I've since switched over to PC (Red ring -__-) and so I'm going to go through again with the same choices, but I'll pick up the DLC as well and get my "canon" run ready for ME3.

Edit: Suicide Mission roles;

Legion in the pipes,
Garrus as the fire team leaders,
Samara as the Biotic,
Tali escorted the survivors,
Garrus and Samara as my final squad.

"Cheated" on Liara and hooked up with Tali.
I consider this fully justified as Liara pretty much dumps you when you find her (Pre-LOTSB).

ME1 choices:
Rachni spared,
Wrex lived,
Ashley died,
let the council die to assure Sovereign's defeat.

Imitating Canadish for this one.

Kept the genophage data "just in case"
Counter brainwashed the geth aswell, was a hard decision but mass brainwashing seems less bad then total extermination, can't exactly be proud of myself for either though.
Every time I replayed the game, took a different stance with the Quarians, so not sure which was my "cannon" decision but I remember the first time I played I was neutral and tried to keep me nose out of such a big decision that wasn't really mine to make as a human.
Saved Jacob's father and arrested him, all by the book.
Blew up the base, part of the Reapers advantage is that the various civs followed a tech path the reapers set, I'm not convinced the risks was worth the payoff.
Mission Casualties: 0.00% My team were well prepared by the time the reapers took my crew, all team mates performed their job admirably with Garrus as 2nd team leader, Kasumi infiltrating, Mordin escorting the crew back to the ship. Oh and Samara as main biotic.

Only thing of note is that my Shepard doesn't have a romance:

-Ashley died in me1 (kaiden was with the bomb and for the sake of the mission needed to go back for him)

-I don't like Asari that much. I left lliara as last team mate to acquire in me1 (in case you didn't know, she goes insane from being trapped so long+rages at you for having all the answers to questions she spent a lifetime trying to figure out) and never used her, she wasn't part of my team as far as I was concerned. In me2 I outright ignored her once she gave me the info I needed to track down 2 other companions

-I hate Miranda with a passion and felt the game had limited opportunities for me to rp that hatred.
What made me despise her even more is she is all but unkillable in me2 regardless of loyalty and upgrades, and only way of killing her are lame ones compared to the violent deaths others can suffer (she will NEVER be taken by the seeker swarms for example+in my 2nd playthrough I'd thought I'd let her justify herself after she had the arrogance to suggest she could samara's job in holding a biotic field. Which she promptly failed and had to see poor Tali dieing a horrible death due to her incompetence. Oh I hated Miranda even more after that)

-Tali was my little sis, but there was nothing romantic going on between us I felt. Would have felt awkward.

-The only character I might have been interested in...is Samara :D Despite my documented dislike of the Asarai, I adored Samara. She impressed me by her lack of xenophobia or arrogance, her knowledge of humans and her ethical code which though I didn't completely agree with, respected.

That she doesn't fry Miranda alive for daring to presume she is anywhere near the level of an asari justicar biotics wise speaks volume of her patience and self control too. Though would have turned a blind eye in this case.

That she declines your romantic advances made me like her even more, she became a staple of my team from then on and my 3rd in command after Garrus.

Aye she didn't, but had max upgrades and loyalty for her, so the % of her dieing was miniscule.

I fear you might have just gotten unlucky in your game :\

I'm guessing you did the same mistake as me and had Zaeed lead the second team correct?

Also, if you picked the wrong Fire team leader (unskilled or disloyal) then that would be another reason.

That was your mistake only Garrus, Jacob, and Miranda can lead the 2nd fireteam succesfully.

For several parts of the final mission there is a % of the team mate failing which you can increase by having them fully loyal and by having their upgrades.

This is especially important for Mordin who has a relatively high % of dieing whilst holding off the collectors as you take on the boss even with a full team helping him and max upgrades/loyalty.

Gonna list the main sections while I'm at it actually.

Infiltration: Only Tali, Kasumi and Legion can do this. They have a % of success based on loyalty and upgrades.

Team leader: Garrus, Jacob and Miranda are the ones who can succeed here. Garrus needs full loyalty though, Jacob apparently can die too but I never seen it happen and Miranda always survives this.

Escorting crew back: If loyal, succeeds, if unloyal, dies. Easy way to guarantee Mordins survival here.

Biotics:Only Jack or Samara can pull this off. Loyalty or upgrades don't seem to affect anything here.

Holding off the collectors: Every character has a % of dieing here based not only on loyalty and gear, but also who else is helping them (grunt dramatically improves teams chances of survival here). Weaker members such as Mordin or Tali are especially at risk here.

The team you bring with you to fight final boss: Survive only if loyal, die if unloyal.

I have formed alliances with the Rachni and the Geth, encouraged peace for the Quarians (despite not being completely honest with them for Tali's sake), kept the cure for the genophage and have made Grunt a legend for killing the Thresher Maw. Samara has killed Morinth, despite me greatly disliking the necessity. It just seemed the only way to avoid stirring up major inter-species issues in the long run.

Garrus spared his traitor at my insistence, but I gave him a chance to redeem himself by leading the fire squad when through the Omega Relay. Miranda and Jack fought briefly before entering the relay, where I sided with Jack, but I managed to regain Miranda's loyalty. I've also sided with Aria at every opportunity, foiling her oppositions (the exception being when I sided with Patriach). Zaeed did not manage to kill the leader of the Blue Suns, but reluctantly continued his contract with me. Jacob's father was killed by his victims. I refused to return to Spectre status and left myself legally dead.

I became romantically involved with Thane, becoming his 'Siha'. On the eve of going into the Omega Relay, he confessed that he'd grown to fear death now that he has something to lose. I brought him and Kasumi with me on the final assault on the Reaper Larvae. Jack escorted my group through the Collector Swarms (because she's just badass), while Samara was tasked with escorting the crew to safety.

And finally, I destroyed the Collector vessel to spite the Illusive Man.

<Spoiler=DLC Stuff>I took pity on Kasumi, but destroyed the Grey Box for her to avoid her past coming back to haunt her. I freed the 'Overlord' and had him transported him to a facility for rehabilitation. I killed the rogue Spectre on Illium and took Liara to face the Shadow Broker, with delightful results, and immediately brought her up to the Normandy to reminisce, where I revealed to her my heartbreak at knowing Thane wouldn't survive much longer.

I shot some people in the face with a shotgun. Then I used biotic charge to regen my shields and shot some more people in the face with a shotgun.

I had no Casualties at the end, romance with Jack, brainwashed geth, told the quarians, blew up the base even though I would of kept the tech if i wasnt giving it to Cerberus. I am thinking of making another playthrough where Shepard dies at the end because i heard a rumour something happend in the third ME like playing a different character ? i dont know maybe its a lie XD

Just because there seems to be a bit of confusion here on how the squad mate deaths actually work I figure I post a link to a thread explaining on the bioware forum http://social.bioware.com/forum/1/topic/105/index/5855452
It even has a graph.

The deaths are calculated, not chance based. Loyalty and ship upgrades make a difference but squadmate upgrades don't. If you biotic is wrong or un loyal then one of your other squadmates will be taken by the swarm.
I had half the crew die because I didn't go to the base right away.
Edit: Cleared one thing up in case I confuse people.
