/tg/ has made up a fun game called chapter master/inquisitor.
Both of them play along the same lines (One meaning you are chapter master of a SM chapter, the other being an inquisitor) and you use your imagination to have fun. http://1d4chan.org/wiki/Chapter_Master_%28game%29
But with all that being said, a game about bureaucracy would be boring. 40k has to have war--it's int the freaking title. Ergo, like the others, a 40k Total War would be a blast, if there was further expansion into loyalty/heresy, tithes from planets, xenos interaction, that kind of crap. If you do well, your fellows get jealous and will hire privateers against your trade routes, or even go as far to denounce heresy. You can fight back with your personal armies under your house, therefore going into the Total war style of battles, as the Imperial Guard just love lines of infantry (Or it could work out like grand strategies where it just tells you the result of generals).
That being said, you'd need to fill out a lot of paperwork in order to do such. You'd have random inspections from Inquisitors to chance your heresy, or any xenos technology (That provide improvements). You would also need to request attack forms in order to invade a planet (that say, has a lot of materials), but are denied order because of something or another. If you take it nonetheless, you will be punished.

And to make the game seem more tense, (Which can make anyone commit drastic acts) have your main character be alive as an actual character, and, if taken out, you lose.
Orks invading your home world? They'll shoot you down from space, you just can't abandon the planet--fight it out, scrub.
Great job, you've got this large empire, and you're near 150 years old and ailing? You're on bad terms with the Mechanicus for stealing a world they wanted-- I hope you don't really need that surgery
Space Hulk in one of your systems that you want to personally investigate? Your Navigator gets perils of the warp. You find yourself back in the year where you began. Have fun running against an AI version of your younger self.
Eldar offering you tech so they can search a world you own? A fellow lord's spy caught wind of it, and now you have 20 assassins rushing at you. Have fun with that.

Space combat would be enjoyable, as well as expanded worlds (So you have to build campaigns around each planet you land on, with continents and cities to hold). You'd have to appoint generals, priests, techmagus, governors and the like, each of which can provide bonuses to their respective area. Can also apply for aide from other members of the Imperium (Assassinorum, Astartes, Sororitas, Malleus, et cetera), by building their favor (Aiding them, building monuments to them, forming blood debts) or lending cash moneys.

Just a large scale 40k strategy would be all that I like. It'd be grimdark and full of loss, but, hey, WAR BOSS SKULLKRUSHA HAS DECLARED A WAAAGH! ON YOUR SYSTEM
