hahaha i think it's because we've all done something that lead to the accidental death of a companion.

random mercenary, burned due to reckless fireball usage
Lydia, ripped apart by shades during my mission to retrieve the dawn breaker sword
Horse, mauled by a bear
other random mercenary, fell from the throat of the world (was shouted off >: p)

RIP Lydia. I thought you would actually be able to pull your weight in a fight. I thought wrong. Seriously, at least the non-essential companions in Fallout 3 could actually take a few hits without dying,

I can only presume she's dead, since she has been standing around in my undecorated and unused house in Whiterun for about 4 ingame months now.

i have some more to add:
male mercenary- lost after joint wolf/bandit/spiders/sabre cat attack (presumed dead)
value of equipment lost ~10-15k (lots of named stuff)

female mercenary- lost after a joint dragon/twin frost troll attack (very dead)
value of equipment lost ~5k. But i also couldnt give her loot to carry so had to leave stuff

Due to this thread, I don't think I'll ever take Lydia out. She sounds like all she knows how to do is die in interesting ways.

Update: I accidentally killed an entire camp of people that I later figured out were part of a quest. Oops.

R.I.P. Male Mercenary, although I think you deserve it for attacking a mammoth and a giant while I was under attack from both a fire and ice dragon. I swear, watching you get blasted with fire and ice, mauled by a giant's club and crushed by a mammoth tusk all at the same time was just about the coolest thing I've ever seen.

R.I.P. Vigilance, the trained war hound I bought for 500 gold, who then was immediately clubbed to death by a giant.
