The old point and click adventure "Buried in Time" had some really weird deaths, including getting killed in a monty python sketch by having a cow flung at you.

In the new Bionic Commando game somehow a guy I had just punched/killed got shoved through a wall. Just his head was shoved through though, and his body was hanging down on the other side perfectly limp. It made it seem as though I had used the arm to shove his head through the wall.

In Morrowind, I had equipped the Boots of Blinding Speed and I was running around Vivec. I only level 3.

So I was on the top of one of the cantons. I was jumping and running around.

There is a glitch where if your speed is high enough, you can run or jump through things. Since the boots of blinding speed increase your speed by 200...Well, I think you see where I'm going.

I jumped and then went through the floor. I landed on the bottom level and I died.

OLD SIERRA GAMES!
Man there are so many amazing deaths in those. One of my favorites is from "Quest for Glory"

If you are a thief, instead of typing 'pick lock', type 'pick nose'. If your lockpicking skill is over 40, then you will gain some valuable experience (and open your nose in the process). However, If your lockpicking skill is under 40 and you type 'pick nose' then you will insert your lockpick too far up your nostril and die of a brain hemorrhage!

The tonberry kill on that FF13 mission, Zenobia the butcher.

Or, random kills on Halo 3 which are unaccounted for...a phenomenon my friends now call "Halo AIDS"

There was a hilarious physics malfunction where if you jumped whilst going down in a lift, Link wouldn't hit the fllor until the lift stopped, doing massive damage to him :p

In GTA: San Andreas I jumped from a SUPER high building and hit the ground hard, it should of killed me but I got back up perfectly okay, walked a few feet and then dropped dead.

In fallout 3 I was walking across a overpass and fell into a crack. I was left unable to jump back onto solid ground but could prevent myself from falling by pressing forward. After a few minuets of trying to get out the crack I realised the futility in it and just let myself fall. The fall should have killed me but I landed in shallow water that broke my fall but also broke both my legs. With a sigh of relief I began to hobble out of the pool only to find that the pool was infact a Mirelurk nest and home to around six 'lurks. All I was armed with was the 10mm pistol and a couple of mines, but somehow I managed to finish them all off and walk away. Got around the next corner and came face to face with a Death Claw. The game really wanted me dead.
