one of the gta games, think it was san andreos not realy sure it was ages ago.
driving in gta, hit an oncoming car, went through my windscreen, over their bonnet and roof, over a cliff, and landed on a parked cop car below... i survived the fall (i think)
the cop then shot me for touching his car or possibly causing a 3 mile pile up on the free way.

Mucking about in Saints Row 2. Chucked a piece of C4 under the Helicopter I just got out of while standing on top of a Hotel. Was feeling destructive so I detonated. Explosion knocked me off the side of the building, hurling me to the ground. I survived the fall (thanks to a perk in the game that reduces fall damage) only to have the Husk of the Helicopter land on me moments later.
I head shotted myself in Halo 1 (on a non modded server on the Bloodgulch map - has no reflective bullet surface)
You see in Halo theres this thing called "lead" where to hit people you need to aim infront of them hence "leading" your bullets dependant on your ping.
I was aiming to head shot a hog driver (with sniper) then suddenly I died. Turns out as I shot I passed one of my teammates (friendly fire was off) and I shot the "white zone" on the back of them, theres a place on the back on the armour were you can shoot and it reflects back, try aiming at a dead body with the automatic rifle (easiest way to do it)
Basically the shot was a 1 in a billion chance, for it to hit with the sniper on the exact ~250 ping lead.

On elder scrolls morrowind I looted a nice robe from a cave mans body I weent out of the cave and it was dark so rested to kill time until it was light but the robe was a vampires so when the sun came up I died and when I reloeded the game the sun came up again so that file was lost
Assassins Creed 2. I was prowling the streets hunting guards when randomly a robber fell on me from above a few moments later ten to fifteen guards dropped on me and pushed me through the floor which respawned me in the air... at least the landscape was pretty as Ezio flailed wildly for forty long seconds.
