In GTA4 on the last mission there is a cut-scene where you fall off of a helicopter into the water below, for some reason though I just flat out died when I hit the water.

In an attempt to avoid/trick an angry civilian into getting run over, I jumped over a divider and onto the freeway, where a taxi slammed into me at full speed, didn't stop, and then slammed me into the taxi in front of him who had stopped for some reason or another.
I laughed. Very hard.
And while not technically a death, while taking Kate out on the mandatory date mission, I slammed into a car while on a bike, sending both of us flying. About two seconds later, I was alerted that I left her behind, despite the fact that she had gone rocketing nearly directly up. I had to restart, so it kinda counts as a death...

I was in Super Smash Bros. Melee and I was fighting as Kirby against Mario (I think) on the Rainbow Cruise level. At the top of the level I grabbed him and did the upward toss which has the special effect of rocketing Kirby into the air. However I must have been high enough so that move sent me flying away "Team Rocket" style into the sky... if I remember correctly my foe fell back to the ground.

Morrowind on the Xbox. I was in the mages guild in...I think it was Vivec. I jumped down the staircase, fell through the floor, was falling for about fifteen seconds, then landed at the top of the staircase and died from fall damage.
That is actually kind of funny as on Highrise I was walking towards the window on the left side of the Ranger spawn (a few minutes after the game started) and as I got reached it someone climbed up and facing the out of the window I fired a shot from my Barret and it completely missed the guy who was filling up my pips and got a headshot on someone at the Spenstaz side....I was promptly knifed thereafter.guyroxorz said:

1) In Tomb Raider: Underworld, I was on the last level and running down a hallway. I hopped over a hole and continued on but out of nowhere, Lara keels over and dies. Before I can say "What the--" I'm already having my last checkpoint reloaded.
2) In Army of Two: The 40th Day, my brother showed me a part in the last level where you can run through the wall and pick off a few bad guys through said wall. What he didn't mention is that if you walk too far off, then you fall into nothingness but you don't die. On the plus side, I got to sit back while my brother did most of the work.
3) This one's more on the hilarious side. In Halo 3: ODST, I was playing Firefight with my brother and one of his friends. We were in a warthog, riding around on jagged rocks until we fell off the ledge. I hopped out of the Warthog prematurely and I hit the ground first. I looked up and saw the warthog crashing down on me. My brother has a screenshot of the vehicle right above me--kinda like when a boulder is about to fall on Wylie E. Coyote.

Pressed 'B' in bad company two and somehow managed to get into a helicopter that was part of the background which dragged me outside of the map.
I was playing Mercenaries 2 and when I got in a helicopter it immediately took off, flipped upside down, crashed, and blew up.
Edit: More recently on COD MW2 I was playing Spec Ops and on the level where you have to get from one end of the bridge to the other I killed every enemy and as I walked to the end a car that was on fire blew up next to me and killed me. I literally killed every enemy and still died.

A tie between suicide by immolation and suicide by drinking caustic fluid for no good reason at all in Shadowgate. Suicide by gun in DejaVu just seems tame by comparisson. That was the kind of freedom that you don't find in games today.
