Getting killed by your own care package crate in MW2 is always fairly stupid.

One match I called 1 in,crate lands,I run up to it,at that moment the crate decides to topple over very slowly(like snail's pace slowly)and kills me

Another match I again called 1 in and hid in a corner about 20 feet away.Crate lands,bounces and flies across the screen and takes me out

I was watching a friend play GTA IV online. I think it was a game where there were 2 teams. The whole enemy team was on a boat and the other team was scattered about. An ambulance randomly came along, I can't remember if it was another player or an NPC that was driving it. Anyway there was some lag, then the ambulance for some reason shot off really fast straight towards the guys on the boat and killed them all. Funny as hell.

I accidentally hit reset after completing the cave of ordeals in The Legend of Zelda Twilight Princess before I got a chance to save.

What do you mean that doesn't count? Fine, playing Halo 2 with some friends, I will repeatedly commit suicide by jumping off the map, where they hit me with one bullet and get credit for the kill. Now, my suicides are either fusion coil or grenades.

After a mission in gta4 I had only a little life left, so I wanted to go to the other side of the street to eat a hot dog. Suddenly a car hit me, it wasn't even driving fast, just a bit but that was enough to kill me. I had to do the whole mission again.

That seems familiar.

Whenever I play Tyro Station on GOW/GOW2 I commit suicide by train.

In assassins creed 2 I was leading a group of guards on a chase. I had barely any health left as I jumped to another rooftop a guard tapped me Ezio went limp plummeted to the ground and grinded down the street.

In a local multiplayer game of Modern Warfare 2, I actually respawned right in front of (like crosshairs perfectly on me) a friend who was stopping to check a corner in the middle of the map.

Definitely Oblivion. I went for a quick swim, but when I surfaced, the breath bar kept lowering. I ran around frantically, trying to get my character to breathe but it just wouldn't happen. I eventually let out my final choked gasps on dry land, after holding my breath too long.

Once I was playing Sly Cooper 2 and there was this massive glitch, if that counts (it did kill me).
I was on one of the missions where you tail the bad guy and try to find out things about him, and I jumped down onto the street where he was. The camera angle failed to show me the guard that was down there, and he went to shoot me. i jumped up in the air, and all of a sudden his gun went off and hit the bad guy right in the face. He began to fly through the air, stuck in one position, shouting gibberish (probably due to the glitch) and fell into the bay next to where he was. It then told me I failed because the bad guy had discovered me.
The funny part is, the point of the game is to kill the bad guys, and I think I did a pretty good job of killing him on accident.

Various forms of accidental suicide would work for me on this. Holding a grenade for too long in MW2, having my own mini-nuke explode on me in Fallout 3, or various other means where I misjudge distance and kill myself on a fall that I believed would turn out ok.

stealthing around that deathclaw infested town in the north of fallout 3 and i thought it would be a good idea to hit one with a baseball bat...
or the old multiplayer on F.E.A.R. had the TK = suicide thing on, sprayed at a team mate, took damage, turned around looking for the source and sprayed at another team mate. it had been along night ok!

Deathclaws are little girly enemies, try taking on a few Albino Rad Scorpions or Feral Ghoul Reavers.

Speaking of F3 is was doing Blood Ties and walked over to the edge of Arefu to see if the river was under where I was, it wasn't.

That happend to me in that X-men ARPG (quick, somebody, give me the name) I thought I saw a healing-thing and jumped down to get it.

Have you seen that video of the guy killing the adoring fan by shooting arrows in the air and having the Adoring Fan stand where they fall down?

I did that on myself... oops.

I call lies. I shot an arrow into my face and it did no damage!

OT: Running out of Vault 101 at low health, I get blinded by the sunlight and accidentally jump off of the lookout point. Whoops.

Throwing a plasma grenade at a wall in Halo (aiming at a person) having it bounce back and stick me then kill me.

That's the one that jumps to mind first, at least.

I know, but since my in-game character was male(could be assumed but...) I meant that a Succubus works for male characters and an Incubus works for female characters, they're different monsters in Nethack, probably to avoid bugs when players are 'playing' with one.(e.g. female characters capable of interacting with a Succubus or getting ignored by an Incubus)
