I nominate the original TMNT game for the NES. The respawn rate of enemies was fucking insane. And don't get me started on that fucking water level.

Chalk up another blood mark on the wall for Battletoads. Twenty fershlugginer YEARS, and still not a prayer in hell of my getting close to the end. This is coming from someone who has beaten The Lost Levels, every Ninja Gaiden, every God of War except the last one and every 2D Castlevania. It's madness.

In general, I would think the majority of this list would be dominated by old-school games such as Ninja Gaiden or Zelda 2. They call it 'Nintendo Hard' for a reason down on TV Tropes, and beyond that they've had far longer to demonstrate just how difficult they are by keeping people of all ages up at night for decades and more as noted in my submission. If a game can continually remain challenging despite people having so much time to formulate and post strategies, well...

Of course, there are other degrees of difficulty. Technically all the old arcade games are impossible since there's no ending, and I've seen ROM Hacks like Kaizo Mario or Rockman No Constancy* that put the originals to shame in the difficulty department.

*- Despite its absurd difficulty, I recommend this one to anyone who likes Mega Man.

Geometry Wars Retro Evolved 1 - it starts out easy, but gets ridiculously hard.

I have and did. I have seen some clever things to make Mario hard that don't involve precision jumps, invisible coin blocks, or the dubbed Kaizo Trap.
Do you mean like the vip hacks? They are not cheap like the kaizo stuff it's all just in really difficult level design that's still doable with tool assistance.

OT: System Shock 2, Far Cry when you meet the tridgens, Devil May Cry 3, SWAT 4 and the Silent Hill games when they are on the highest difficulty.

I Wanna Be the Guy: The Movie: The Game

It's all about trial and error, which starts you pretty far back since checkpoints aren't too frequent (unless you play it on normal but then the game criticizes you, if memory serves).

And I know someone's gonna quote me and tell me I'm wrong, but Demon's Souls. At the start of the game it's fairly hard, but doable, still beatable, but when you play a new game+ (using your previous character), unlike most games with this feature, the game gets harder (which is cool, though). I find a lot of the things make this game difficult. Like dying and dropping all your souls...

I thought it was fine actually: It was just the right level of challenging apart from one or two insanely frustrating spikes (Horizon, Collector Ship ambush, fighting the 3 YMIR mechs etc).

After the midway point it becomes a lot easier with all your upgrades. The worst part was that your team-mates were so thick, I ended up managing every single action they took, playing the game like Dragon Age.

The hardest game I've ever actually managed to beat without cheating was Gears of War 2 Insane mode - playing solo.

It's not too hard with a buddy, but alone it is a fucking nightmare, especially during the on-rails turret sections... fucking Reavers.

The Gothic Series has a reputation for being -very- difficult at the start, though it gets more bearable later on.

Well, how about Abadox? I know that game handed my ass to me on a regular basis as a kid.
The Mega Man games I could eventually beat, but man-- Abadox was a *****.

I remember Shadow Warrior on PC kicking my ass pretty hard...

Batman on the NES was one of my favorite games growing up, but I don't think I ever beat it.
Some of those later wall jumps required insane precision and timing.

I'd say Ninja Gaiden 1. On it's hardest difficulty i've heard only like, 12 people can beat it? Something like that?

Its not nearly as crazy as some of the spaceship top-down shooters others have posted, but its extremely difficult and its honestly a fair challenge. As in, it's hard as hell, but you never feel like its impossible. You feel like you just need to get better and learn from mistakes.

Ninja Gaiden 2 didnt seem as hard to me. Well, until the marine guys show up like in every Ninja Gaiden game...ugh...they're like the flood in Halo. Annoying and terrible. >.>

other than that i can only think of wind waker because it gets so fucking boring with the triforce collection. Never even beet the dam thing.
