I know that sounds excessively pedantic but really, a few quality minutes with Google and some reference pictures would solve it. Hence: They Just Didn't Care.
It really, really does.
Fair enough, but I'm not going to feel bad about it because this is supposed to be the little things that bother us. :p

Quick example, and continuing with the gun-theme:

I put 'M-16' into Google, first link is Wikipedia [http://en.wikipedia.org/wiki/M16_rifle], of course, which give me pictures of it showing the overall layout and operating scheme. Also, it tells me things like that it fires the NATO 5.56mm round and that most standard magazines have a capacity of 30 rounds.

That took less than two minutes. If your game is set in a modern 'realistic' setting you have NO excuse not to spend the little bit of time it takes to figure this shit out.

Again, the peeve here was: They Just Didn't Care.

I think this bothers me as much as it does because usually it feels like that if they couldn't be bothered to do something so basic and easy as this then the overall polish level of the rest of the game is usually about the same.

That is to say: Utter shit.

EDIT:
Fixed a typo.

But even more amusing is that James gets killed less than an hour later, making your entire rescue pointless.
ALSO THIS! Oh my god I get pissed when that happens, like in MW2, after you escape the house, you get shot in the face and burned! Like it would have made a difference if I had died IN the house, can't shepard just walk in and take it?

WHY DID HE KILL ME ANYWAY?! IT DOESNT MAKE SENSE! He wants more people in the military, so he shoots me in the face? I never objected to his plan! Hell I would have gone with him!

I hate how most shotguns are only effective up to ten feet, when they have a greater range in real life.
This and the sniper thing, it pisses me off no end.
what does it have to do with snipers? and yes, i agree with the first one. if you can see a shotgun's spray and a lot of the bullets were heading towards an enemy, they shouldn't just disapear if they're two feet too far away

When kissing in games looks like two mannequins falling over on each other and when female characters wear less clothes than any sane person would and still perform well in combat and never get cold.
You'd not like KotOR's romance scene then.
Fortunately there's a mod to fix it though.
OT:
I hate it when they have a small ring of voice actors in games with huge worlds (I'm looking at you, Fallout 3.)
I mean c'mon, you KNOW you're making a game with tons of modules with people, why the fuck would you hire FIVE FUCKIN' PEOPLE?!
At least deepen the pitch of the person's voice in audacity or something, man!
I hate stupid ass scenes like in the Godfather the game where the character doesn't even shed a fucking tear when their lover dies and practically go emo when their best buddy that they haven't really seen very often in the past few months dies.

NO LOOT FOR YOU! So a game involves shooting things, some of these things you shoot also have guns, then after you shoot them the guns they had either disappear or you are just not allowed to pick them up. LAME. If it was on their person, and if it is something you could conceivably use, THEN YOU SHOULD BE ALLOWED TO PICK IT UP. I know for this issue people will bring up 'Game Balance' and 'Well, if you could pick up his radio and call for help, there wouldn't be much of a story then', but I call bullshit on that. If your story or game requires artificial stupidity then you are doing it wrong.
I hate that one too. Like in halo when you can never pick up that awesome gun that the hunters have. Or in firefight when the guns just dissapear. Whatever happened to storing guns for later when you really need them.

Let's see...

Any new FPS where you can see your character's feet. Why? It's unimmersive.

Let me elaborate. When, in real life, you look down as you walk, you don't notice your legs and go "OMG there's my legs! And they're moving!" You're sending the muscle signals and are constantly "aware" of exactly where they are. You might actually say you don't "notice" them at all. That's why they're not needed in an FPS for immersion. People aren't immediately aware of the fact that they're not there, and it's just more design work anyway.
Allowable exception for Mirror's Edge, since seeing your feet was sort of necessary to get a good perception of where to jump.

Well, see, he wears that armor. Which means he doesn't have convenient access to the magical space known ass "The Rectum" Where many characters, such as Link of Legend of Zelda fame, keep all of their items.
That magical place is useful for storing all sorts of items. I use it all the time

Even works for teammates in some games, although back in the days of the Ps1 we called such a thing human trailers.

Ok I see your point but when something is more or less part of your arm, its kind of hard to rip off in the heat of battle.

I understand the whole "the gun is no longer there" is bullshit as well as the radio. But, again the radio is kind of hard to get to and at that point itys just another "search enemy" section that other genres have. It removes immersion because if it was there, they would either have to stop time to let you scrounge around in the dead guys stuff, OR let you get shot; which will immediately cause some bitching. The radio would be a nice little useless add-on though saying something like press(button) to call for back up, and the response would be something like,"there is too much AA in the area. Too fucking bad for you."

Just because its useless, doesn't mean it can't be done. Hell, let me have reinforcements at some points when its understandable.

Games like GTA 4, which was good, but you got this annoying ignorant immigrant with a really annoying accent and he looked over-weight no matter what clothes he wore.. That and no jump buttons, stupid AIs, and when you can't choose what your character does like if its a really stupid ass plan or you fight because you disagree and your such a goody goody thats really annoying...

Crappy auto-saves, you'd exspect them to save at a certain point in the game that is important/hard or ever 5-10mins or so. But when its every 1-2 hours!!! gets annoying when you have to repeat a task that takes you 10-15mins. (I'm looking at mass effect, still a good game though)

When, in a shooter, you're bullets disappear when you die.

Because of this, I can be shooting at a guy, and if he kills me first, my bullets disappear, meaning he doesn't die like he would in real life.

Games also do this with claymores and C4 but it's not quite as annoying.
