Lag is annoying as hell. I also have a strong dislike for unnecessary over-sexualization of women, as seen in nearly every game these days that contain women. Notable exceptions which I appreciate include Half-Life 2, the Halo series, and nothing else that I know of. Even Pokemon seems to have a bit of this, which is disturbing considering half the people who play Pokemon are kids.

Unskippable cutsecnes or dialogue you have to sit through several times over until you get past that part. Automatic or badly times saved points. As in a game saving while I have a sliver of health or the game just happend to save as I'm falling over a drop off to my death.
Yep thats exactly what I had in mind. I remember how annoying some of the dialogue was in some of the FF games or that bloody owl in OoT, if you accidently ask him to repeat himself, I just wanted to choke him!

Really long unskippable dialog. Like someone said, the Owl from Ocarina of Time: you keep pressing the button to go to to next box and then at the end he asks if you want to hear it again and if you're still pressing A, then he repeats himself. All. Over. Again.

Gonna have to go with these two:

I can see how elevators can be annoying, but they are no match for invisible walls. Hands down the worst thing ever in gaming.

Oh god those are so annoying in Fallout 3. I'm just exploring when suddenly I smack right into a barrier that tells me to go away. It's the perfect way to break immersion. They should have just hit you with a huge amount of rads every time you wandered off the map. That would at least feel more natural.

Oh yeah, the game can also spawn enemies outside the invisible walls. So if your a melee character, all you can do is run away whilst being shot in the ass.
It's a great big gravy stain on the white shirt of immersion.

I never knew tanks could do it, since when they punch things they send them flying... Wait, actually, it would be impossible, since the tank wouldn't spawn until they got to the top of the elevator. They used to have something where you could be a boomer and punch a generator or forklift in the way of the elevator, but they got rid of that.
Anyway, for me, the most annoying thing in a videogame is a long combo of hits that you can't get out of. I know it only happens in fighting games usually, but it gets really annoying when you fail to block once, they start hitting you, and you keep trying to escape it while they take a good chunk of health.
Grinding is a close second, and I would have to say, invisible walls are annoying, but only in certain circumstances/games.

Oh yea. Good call it was the boomers. I'm glad they got rid of that. It ruined the game at first.

Lives. Yatzee has a very valid point in saying that the lives system became pointless the moment we purchased the games and stopped paying quarters per play.

I'd have to say shields that surround your armor like in Halo 3. I hate unloading two magazines on a guy, only for him to turn around and kill me with a blow to the chest.

I hate the games that wont let you harm children. Heres looking at you, Fallout 3!

That stupid brat from Greyditch ran up to me, and unfortunatly for him I was on my newly started evil character. I was so disapointed when I told him to buzz off, started shooting him and he just ran away, endlessly absorbing bullets.

*Edit*
This has actually got me thinking. What the hell is the point? Its like they are saying violence against children is going too far, but its only virtual. Killing a child in a game is not going to make me into a child abuser, in much the same way that killing any other NPC wont turn me psychotic. Stupid developers :<
Behold, a mod that will turn every child in the game vulnerable to your onslaught.

Of course, if you're playing on the X360, you're out of luck ;)

Escort Missions. Half the time the AI of the person you're protecting will take their time or wander right over to the enemies. It also doesn't help when the enemies seem to have the whole world conspire against you to get your allies killed.

By far the worst, is when the publishers expect you to use guides or walkthroughs.

I'm going to have to say something that probably most people wont agree with, but too easily skipable cutscenes that you can't watch again without reloading a old saved game! It really annoys me when I'm into the story and nudge something wrong on a control by accident and miss out on a important part of a story. One game I played skipped when yuo moved the analog stick at all, that was ridicules.

All cutscenes should be pausable, with a skip option in the pause menu, checkpoints after each cutscene so you're not forced to wait for it to load every time you die, and there should always be a theater option to rewatch seen cutscenes to refresh yourself on the story.

The health bar concept of damage modeling as employed by "fight" games. If I hit you in the leg with a lamp post, and you're not the Thing, you'd better start limping.

Poor pathfinding in the AI.
Culprits that I can think of are most RTS games, ARMA(1+2)/Opflash (re: ARMA 2 there is a clip somewhere of a T-90 MBT getting stumped by a load of chickens in the road, either go around them or go over them, a T-90 ALWAYS has right of way).

Eh, Turn Based Combat and way to much text you don't care about... *cough* Hotel Dusk *cough*
I do love turn based combat when there's a good reason for it, Fallout or Civilization come to mind. When its badly implemented like Battle for Middle Earth or Arcanum, then I'm with you 100%. (Also if there's tons of text, at least write some funny stuff in there for those OCD victims like myself who will actually read it.)

I hate "no quick saves in combat" ("in combat" meaning an enemy who saw you once in high school is two miles from your present location) or "save only at checkpoint" schemes. Really guys. There's a sequence in Mass Effect where you have to hit precise points with grenades, and you can only carry enough grenades to miss twice (IIRC). The sequence isn't that hard otherwise, but its very VERY easy to run out of grenades. Top it off, it's actually about four combat sequences, but the game WILL NOT LET YOU SAVE between them. (And yes, I do know about the extra grenade pack, and that technically the grenades tactic is optional, but it's still bull---- difficulty.)
