Crushed by the destroyed wreckage of a giant tower I knocked over in Red Faction: Guirella.

Somehow running directly away and not off to the side seemed like the better idea at the time*Squish*

Done that one too.

Of course, one time when I did this with a cylindrical tower, part of it rolled after me. My friend who was watching said I was the Martian Indiana Jones.

My own overcooked grenade... In F.E.A.R. 2. They even show you the timer for the grenade in that game >_<

I was playing GTA 4 at my friend's house. My health was down 30-40 percent because of a shootout with the cops. My wanted level was back to zero, when I was just walking. I tripped and fell down a flight of stairs. Yes, I died. I was killed by a staircase.
