George W. Bush entering office. TWICE.
Seriously. When his second term came up, I was like, "There's no way he'll get elected again." And I went to bed feeling better.

And then I woke up in the morning to, "WTF OHIO."

Clearly the worst fail is letting natural selection be overridden. It would actually fix a lot of your answers.

well what do you think it is?
Can be anything! real life, in games, films ANYTHING!

I'd say who ever predicted the millenium bug YK2 worry, oh dear.
Well that isn't as "epic fail" as you might think. A lot of people thought that dates were stored as 2 digits rather than four. You can see the mix up. What you should say is whoever hyped it is "epic fail". However, the 2038 bug will be real and not deserving hype but attention.

I was flipping through channels one day, and I landed onto this one specific moment. A guy had his hand stuck in a garbage disposal and was surrounded by a ring of fire with the power flickering. And there was no one else in the room. I wanted to look in the window of that scene and impersonate Cuddles and hold a sign written 'Epic Fail'.

Digger and Friends. NASCAR's attempt at animated comedy. What pisses me off is someone actually got paid for this. >(

In arguing against an American national health service, a political (can't remember who) said thus:

"If Stephen Hawking was born in the UK, and had to go to the NHS when his condition began to affect him, he would not be with us today"

If you cannot find the fail in that quote, you sir, have also failed
