In the last UEF mission of Supreme Commander, when the little Monkey Lord brigade shows up on your island.

For those unfamilar with the game, the MonkeyLord is one of the faction's super-weapons, and probably the signature unit of Supreme Commander. It's very large, fairly fast, amphibious, has a lot of armor and sports a variety of weaponry, including the Microwave Laser which would do somewhere on the order of 12,000 damage a second (enough to kill any unit aside from other super units, SCU's, or upgraded Commanders in under a second). It was a very cost effective and threatening Experimental.

But anyway, the whole mission is defending an island (technically half of it, since the other half is out of view) and things aren't that bad. The mission is mostly fending off large air and sea attacks and launching a sea based attack on an enemy islands, so I ended up with essentially a U shaped defense network of SAMs and Torpedo launchers, with the very north fairly open. Then comes a very large attack lead by a number of MonkeyLords from the north part of my island.

There was no reason to defend that area with anything, let alone ground defenses that could hold off multiple Experimentals. I was just dumbfounded, it was outside of my box of thinking, it wasn't something I was expecting or at all prepared for. Just left me stunned.

They ran though half my island as I proceeded to nuke-spam my own island, since I had virtually no ground assets to actually combat them with. I stopped them before they destroyed my defense objective, but lost most of my economy, most of my defense structures and most of my SCU's that were doubling as builders and resource generators. Disastrous.
That's why you ALWAYS build ground turrets, no matter the situation. Besides, ground turrets can shoot naval units as well. Mavor FTW.

One that I can remember is from Supreme Commander as well. Playing a skirmish against an NPC/AI player and built up a nice base defense, wall around most of my base with tech 1 + 2 guns, tech 2 artillery and anti-air guns along the edge facing the enemy base along with shield generators covering the whole lot. Wave after wave of vehicles came at it and got wiped out, mostly before they could get a shot off. Then all of a sudden an Aeon UFO Mothership (forget what you call them) looms out of the fog towards my base, with flak guns blasting away at it I just manage to take it out before it can do too much damage to my defenses. Unfortunately it didn't do what I expected it to do and blow up in mid-air but rather plummeted out of the sky, flattened two artillery guns some flak guns and five or six point defense guns. Not too much damage until it went nuclear and destroyed another twenty guns and three shield gennys. Taking out about a third of my defense in a few seconds.
Those godly machines are called Czar's. It's probably my favorite experimental unit from that game. 1, as you saw, was bad enough. My plan usually involves 3. With a full compliment of gunships that come out of them.

That volcano armadillo in Ninja Gaiden 2. Anyone know what I'm talking about?
So I kill it, y'know? Slash it with my sword and shoot it with arrows. The thing is out of life and suddenly explodes like a nuclear bomb, killing me.
"No fair!" Yelled Tyson. "How was I supposed to know it would do that?!"

Gears of War 2, Last Act on Hardcore. The part where you have to shoot down like 8 Reavers to defend the communications dish. Maybe I'm just stupid but I tried that THIRTY times and couldn't do it. I finally gave up on the game for like a week until a friend helped me out and we did it in one try.

Had to replay a level like 5 times because the game won't save correctly. That was pretty unfair.

Damn their spikiness to the fiery pits of the abyss!

Also: Through the Fire and Flames on Expert, Turbo Tunnel, Malak's regenerating health in KOTOR (though that was easily solved after the initial shock), and Tetris on a high degree of difficulty.

Star Ocean: Till The End Of Time (or SO3 if you want): The first time I fought Lenneth and Freya, I got absolutely steamrolled by their bajillion hit attacks. Seriously, 20000 x 20+ damage is not fair.

Games like Advance Wars where the computer AI can see through the fog of war. I specificaly destroy your recon troops so your long range tanks can't shoot me but the AI cheats and has some kind of crystal ball divining me presence.

Actually, the AI can't see you when you're hiding in trees or reefs. Although it might be that they can't target you in those terrains, since you can't see anything in trees or reefs unless you have sometime right next to it (or you use you're Power/Super Power as Sonja).

The final boss of Dante's Inferno has an attack that is literally no fair. He makes 2 little tornados that if you can dodge the first time, don't disappear right away every time, and you have to dodge them again, while also jumping to avoid his ground pound. Oh, and the tornados can catch you even if you're in the air, so it's which does the most damage and which is most inconvenient pretty much...it's the worst thing ever...so not fair...

Any game of left 4 dead where you get smokered, boomered then huntered in less than a minute.
Left 4 dead 2 where you get a horde Literally every minute and a group of at least two special infected also every minute, on advanced, with useless teammates (this was Co-op not VS fyi) It was all going O.k until we got into the Mall itself on Dead Center suddenly I swear that all the spawns were in overdrive, we literally got THREE hordes Before Even Leaving The Saferoom -_- That pretty much set the scene. After rescuing and reviving all three of my teammates for the fourth or fifth time that game and less than halfway through Im just like "Screw this" and left.
*breathes in*
Also S.T.A.L.K.E.R with the rebalanced mod. Ok lets set the scene: I had just got into the the Chernobyl NPP and was taking it slow when suddenly a group of 6 heavily armed military killed be from behind. SO I re-loaded and took it quicker, taking the Monilith out quickly and sprinting out ahead, only to be owned by the Choppers overhead. To cut on length, I ran like Hell and used like 20 medkits (out of the 50 or so I had brought >..< At the end of this I was down to about 30 guass round left and 200 SMG ammo and 100 or so AR ammo. Once the cutscene finished I could continue playing (due to the mod) so im like "sweet just gotta get back to Freedom and rearm etc" Only to relise that I had to get through the teleport zone with its leftover dudes....Chernobyl NPP which was still a battleground, Pripyat which with the mod was FULL of mutants and Monilith. I finally got through these trials, and even found a friendly band of loner at the end of pripyat. Full of hope I prepare to sprint back through red forest to safety, and Once The Loading had Finished!!! I was shot in the face by 5 exosuited guys who were sitting right in front of the level spawn. Fortunately the game crashed when I died, saving me the effort of quitting. *Breathes out* Phew.

Not really a "moment", but I hate the fact that, in most Final Fantasy games, really strong enemies are almost always immune to most status effects, while the weaker enemies that aren't immune are easily defeated without inflicting status effects.
