Yeah, but in a subscription game, those upgrades are obtainable in game at no extra charge, save for one's time and/or in-game currency.
Ideally, yes. But not all of them. *cough, cough [http://us.blizzard.com/store/browse.xml?f=c:5,c:33]*

I think MTs are a legitimate way to run a game and in the ever-growing MMO market, it's almost a requirement that many be F2P because most folks won't have more than one sub at a time. However, it can be frustrating to be nickel-and-dimed over every feature in the game, especially if it's one's primary MMO and therefore played often.
Hence, the "potential for evil" comment.

"Why not have a F2P/sub hybrid?" Because then almost all "non-essential" items get relegated to a cash shop, even for the subscribers. I guess some folks just prefer a simple, straightforward payment plan.
Moving further off-topic (heh)... Jef Reahard over at Massively.com would agree with that [http://massively.joystiq.com/2011/08/30/the-soapbox-subs-and-cash-shops-two-great-tastes-that-taste-a/], which is one big reason I don't want to play The Secret World anymore.
Coincidentally... He, like myself and many other jaded fans, think SW:TOR could have been better [http://massively.joystiq.com/2011/09/13/the-soapbox-why-mmo-combat-sucks-and-how-bioware-couldve-made/]. That's not to say it's bad or going to fail or anything like that, just that it had some unique opportunities to evolve the genre that were passed up (on purpose, I hear). We'll have to wait and see how that turns out.
