I have 3 that come to mind. The Metroid Prime Trilogy Special Edition, Radiant Historia with Soundtrack, and Tales of Symphonia. Probably not overly rare because I actually like to play games I own, but it took me a while to find each of them.

The original Kingdom Hearts is apparently "super rare." The site gave it an 84%. I might have something more rare, but I can't really be bothered to search up all of my games.

Also, this site is missing quite a few games.

I have one of the misprint FF7 Black Labels.
Not super rare, but far from common.
I have some imports, and older, less common Dreamcast games. But most of my unique stuff is hardware.

I've owned lots of rare Saturn and Dreamcast games in the past, but most have been sold to save for my trip to Japanese. I do, however, still hold:

I have a copy of Final Fantasy Adventure and all three of the Legends games, but that's about it.

I have that too! Not new of course. The case is in good shape, but the Instruction manual is missing in action.

I also have the Zelda Collector's Edition on Gamecube. It still kind of peeves me that they messed up on the sound and frame-rate for Majora's Mask.
You know what the funny thing is? My copy actually came with two instruction manuals.

You can navigate by just clicking where the cursor turns into an arrow. Click the edges to turn and in front of you to walk. It was pretty basic but it was also very vague about it's puzzles so you had to figure out what the rules were.

Yeah... the computer did most of the moving for me. The puzzle areas were pretty rigid.

That said, those puzzles were fantastic.

(Except the Hall of Hidden Links. That one can bite me.)

I still own Rygar (NES), Gyromite (NES) and FF1 (NES)

oh and Extreme G (N46). I don't know how rare any of them are, but of them all Extreme G is my favorite, only cause it is the only game in existance that has a better soundtrack then Super Metroid (SNES)

Pikmin 2 for the Gamecube. Just getting the disk cost me something like $60, and good luck if you want to try and find a copy with the original box art or the instruction booklet. And no way you're finding one with both.

I suppose that goes to my in box Guardian Heroes, looks like it's pretty up there in rareness despite being re-released in XBLA form.

According to that site my rarest game is Pokemon Crystal. Which is kind of suprising given that pokemon is no where near that obscure.

Has anyone here ever heard of Pet in TV for the PS1?
Because I own one of the few copies that exist.
Now that is some obscure shit right there.
Honourable mentions are Suikoden 1 and 2 (PS1), Okami(PS2), Kingdom Hearts 1 and 2(PS2), MGS3(PS2), SOTC(PS2) and others that are probably rare and I just don't know it.

Well lets see. I have Tail Concerto and Devil Dice for the PSX, NFSMW Black Edition, ZOE 2, Killer 7, Ultimate Spider-Man Limited Edition, Battle Engine Aqualia, FFX which is apparently super rare for some reason, FFXII Collectors edition, God Hand, both Gungraves, Ribbit King with bonus disc, Shadow of the Colossus, DBZ Tenkaichi 3 with bonus disc, aaand Wild Arms 5 10th Anniversary Edition. Whew.
For PS3 I have the Bioshock 2, inFamous 2, Castlevaina LOS and Uncharted 3 collectors editions.

I still don't have half the PS2 games I want though. COLLECTING!
