I think is Nemesis (RE3)he can chase to trough doors he appears out of nowhere and you never expect him.
Later on the game he gets even more powerful if you dont have a granade launcher,a missile launcher or at least a revolver you dont stand a chance against him

idk about the scariest but one that comes to mind is alex mercer from prototype. he's pretty freaky.

oh and pyramid head from silent hill 2! he himself isn't all that scary but the atmosphere of the game plus the fact that you know he could kill you without trying and theres nothing you can do about it is pretty terrifying.

Nemesis was pretty freaky but for my money it has to be Shodan from System Shock 2.

I refuse to think of a being carrying around a great knife about 6 feet long that routinely rapes mannequins and has a heavy, metal pyramid atop their head to be anything but absolutely fucking terrifying.
