Of the ones already listed:
Viva Pinata: I got it free with my 360 (as I imagine many did) & think its absolutely awsome. Whoe knew you could make horticulture fun by simply adding sickeningly cute coloured critters?

Gangland: Yeah its pretty good, I read about Gangland 2 in PC gamer ages ago, but have seen nothing since so assume thats a dead duck.

My additions:
Knights of Honour: I dont know how many people have this game, but I personally think its one of the most fun RTS' Ive played/own.

Gangsters 2: Ill say this much, its not great, but its the best Gangster "strategy"/"rpg" game out there in my opinion.

Tropico 2: The sequal to the great (& well recieved) city sim that noone ever seems to mention anymore. I still play it alot

I kinda wanna add Stronghold, but that game did pretty well when released.

Stronghold and Stronghold Crusader, some of the best castle siege/RTS around and were completely ignored, I know of no one else who even knows about them

Shiren the Wanderer (DS): BEST Rogue-like I've played in a long while (I know I spam this, but it's just that good a game)

Front Mission First (DS): Remake of the first + the other side's story (there's two sides to the war). great tactical combat, but after 3 it kinda went downhill (2d --> 3d >.>)

Terranigma (SNES): A great RPG in the dying days of the SNES, so it got passed over

Super Dodgeball (SNES): Wacky, Jpn-only game that was nevertheless a satisfying experience from beginning to end.

Disciples II (PC): Good game, but TBS is a small market dominated by a few names so forgotten.

Kohan: Immortal Sovereigns (PC): A good RTS game with a level up ability, the latter versions have become mediocre 3d RTS's.

Shadowflare (PC): A diablo-like, it's actually a really well-made game and has almost the same level of addictiveness. It's shareware (I think 1st chapter is free, the other 4 come in packs of two). Also, nothing beats dropping mines and killing off swarms of baddies.

Suikoden I for the PSX. I loved that game so immensely, but it was released in the Final Fantasy era, and thus none i know of has played it. Perhaps it was more popular in other countries? Hopefully...

Other than quite a few of those listed already, I have to add Ragdoll Kung Fu. I loved this game. It wasn't grand or epic in any real regard, but hell if it wasn't one of the most fun and original games I've played in a very, very long time. Others worth mentioning:

Freespace 1 & 2
SIN Episodes
Dystopia
Shadowgrounds (wasn't the greatest, but was really fun with 2+ players on the same PC)
Freelancer
PREY

I know there's more that I've played, can't think of them at the moment.

There was a time when you could have mentioned Psychonauts as a "forgotten gem." But lately, it's become such a cult classic that sure, most people in line for GTAIV might not have played it, but the hardcore gaming community have immortalized it as their champion of under-sold titles.

As for the rest of the ones I left in the quote... ARE YOU KIDDING ME?? Yes, they're great games! Yes, maybe they haven't sold as well as well all would have hoped, but "unjustly ignored" and "forgotten gems?" Prince of Persia and Shadow of the Colossus sold way better than any of their fans care to admit. They have both reached Greatest Hits status and continue to sell to this day, be it new or used. Okami has JUST been re-released on the Wii. Final Fantasy Tactics? Final Fantasy ANYTHING does not belong on this list. The freakin' ORANGE BOX?? "The cake is a lie" is probably the most dominant "secret handshake" of the gaming community since... well, EVER.

Next, you'll mention Pac-Man... lol

I'm not sure if it's overlooked or not, considering the factor of i only know one other person who has so much as heard of the game line, but Guilty Gear XX Accent Core is a pretty decent fighting game, and quite frankly a nice change from the constant "mainstream" fighting games we see out there all over the place.

And now just for enlightenment purposes, (i don't live in japan so i have no clue how well this game is known) but Melty Blood is another unique fighting game that has brought me and the select few i introduce to it quite a bit of enjoyment out of it.

No Orange Box. No, really, game that sold over a million copies not counting digital distribution should not be in here.

The 'Neep's picks are as follows:

System Shock 2 & Clive Barker's Undying
The commercial (not critical, mind you) failure of the former sent it's publisher to the morgue. I believe the latter just killed it's developer. These are easily two of the best, most frighteningly atmospheric games I've ever played, in spite of their flaws.

Vampire the Masquerade: Bloodlines
I'll be blunt here: the game isn't finished, in spite of years of work by it's fans. Even now, in spite of its bugginess and nasty habit of messing up the windows registry with all of it's hard crashes, I'm still tempted to go back and play through it again.

Planescape Torment
BEST. RPG. EVAR. It's iffy combat (Come on, its 2nd edition D&D: What can you expect?) is a very minor fault and totally overshadowed by the staggering quality and originality of the writing and the depth of the characters. Its also the only RPG I've ever seen that offered the best of both worlds: both a strong back story and personality for the protagonist AND the ability to play him however you wanted.

Oh and as End_Boss says, Syndicate rocks. Molyneux mentioned that he might be making another after Fable 2. *Crosses fingers*

3D realms PREY - Great horror shooter game, portals (yes this had portals before portal) and one of the best multiplayer sections (surpassing halo by my opinion) *looks up* "What?!?" *gets killed by guy standing in the ceiling*

LotR:TTT GBA - Great hack/slash game for GBA

Sword of mana GBA - Only game EVER to make me cry over a character cositing of 50 pixels dying. Other than that an absolutely amazing hack/slash/RPG

Darwinia - A strategy game made to make you invent new ways to swear.

I was really young when I had this game and I judged games on a lower standard, but this game OWNED... a random world generator so the RPG adventure followed the same story, but was different everytime. Intuitive Puzzles that make sense - give the miror that makes everything look opposite to the ugly old witch so that she "uncurses" the beautiful village women -, stuff like that.
also, being able to bring another of your characters into the game and play with a party. I think taht was revolutionary for the time.
it was like Diablo Siege except it predated Dungeon Siege.

Everybody has heard of worms.

Here are some seriously obscure titles that were popular, or were part of a popular series that faded away almost instantly. Probably over shadowed by the release of the latest adrenaline inducing popular gore fest FPS.

GTA II. Not so much looked over during its time. But since GTA III & the move to 3d trickery loads of people seem to have forgotten the birds eye view anarchic charm of the first 2/3 (depending on how you class GTA:London). I wouldnt be surprised if alot of people who play GTA now didnt realise the franchise existed before III.
