Ratchet and Clank: 2 things. First opening the quickselect menu doesn't pause the game. Second you can't strafe.Other than those it's pretty much perfect.
Both these problems were fixed in the sequels. Does that mean I think they're perfect games? Hell no.

Jak 1 and 3...umm...I got nothing.

The steam tunnel sequence from Journeyman Project 3: Legacy of Time. It's a very immersive game and generally stays away from being too annoying, but when you have to figure out the correct way to divert steam into a certain room to melt a butter statue (trust me), it gets stupid. The correct sequence doesn't make a whole lot of sense, so without a guide, you're going to wander around the tunnels for hours.

The Waterhazard level in Half Life 2 was terrible in an otherwise outstanding game. And the part in Uncharted where you drive that boat thing up river. They expect me to drive, dodge exploding barrels that kill me instantly and shoot bad guys at the same time?! Preposterous!

And when your escort walks so incredibly slow its almost as if they are trying to insult you.
