1) Don't make the inventory accessible only in real time! This was the main thing with RE5 that pissed me off. In RE4 you paused the game to switch weapons and/or heal, but in the sequel, you have to find that health item while zombies are shooting/running at you!

2) Final boss battles shouldn't be anticlimactic! Lookin' at you Resistance 2. I just killed a Kraken with a Pulse Cannon, a giant bug with shotguns and a carbine, a big-ass four-legged robot, and a Leviathan that makes said four-legged robot look like a chihuahua! The final boss battle should be something more than just running around in circles shooting at electrical orbs!

Spiderman 2 was okay.

Rape. Of the child variety.

Oh, and micro payments. Despise them. Let be just buy the damn game for £20-30 and I'll be happy :/
OMG, who thinks that rape of any type is a good idea? I mean, if quick, yeah. But I just feel dirty and disgusted when I see a scene like that which is just too long. For example, watch "The Last House on the Left"...seriously, yuck. Also, having to collect a bunch of small items hidden throughout the ENTIRE game...that just annoys me because I prefer to 100% completion yet don't have the patience to explore over and over again to find some small out-of-the way item that was hidden in on the side of a wall with no explanation as to why it would be there. C'mon, let's be logical.

Basically, every game should be played early on in its production, and any parts which are not enjoyable should be cut.

Token online. If your online isn't going to be competing on or near the same level as COD and Bad Company, then there is absolutely no point in just throwing some online play in there. The budget and development time could be put towards making a stronger single-player.
I'd take it one step further and include multiplayer exclusive acheivements. Some of us can't access the online multiplayer portion, why should we miss out on the Platinum?

There are a few on the SNES. Lion King is a fun platformer, if difficult. The Super Star Wars series are quality, as is Indiana Jones: The Greatest Adventures. However, all of these games are pretty hard.

Save points.

I think we've moved past that, I'm looking at you JRPGS as you're the most guilty of it. When a game is as long as a JRPG, save points are stupid, you should just be able to save whenever, I've lost count how many times I've left a console on because I had to go out to work or whatever because there was no save point in sight.

Not being able to walk in games. What? I like being able to take it slow and have some kind of "stealth" option that doesn't involve crouching and creeping about. How hard can it be to throw in a "walk" button! Oh, wait. A console controller has only so many buttons... *whisper curses at the console gamers under my breath* No, just kidding. It's the game developers laziness, not you ;).

Boring or unfun first few levels.

really. its common sense.
Agree. This is where the "it get's better 10 hours in" comes sliding in on broken wheels. Why did they not flip it and have the last 10 hours be boring rather than the first hours which are there to get the player interested!

QTEs, silent protagonists, vehicle sections that are harder than driving an actual vehicle, love interests for those silent protagonists (once again, I'll stop ragging on Alyx Vance when she's dead, then I'll celebrate), inverted controls (the flight sequences in God of War 3 being a perfect example). That's all I can think of at the moment.

EDIT: Oh, and random upgrade systems. There is no reason for there to be an upgrade system in Splinter Cell: Conviction. Modern Warfare did a good upgrade system for guns, nobody else has.
STALKER? Or do you mean only games you have played.
That last one.
Mjes, I would say S.T.A.L.K.E.R did indeed to a good upgrade system, so did MW. But no games does an upgrade system that I am pleased with. This may be cause I enjoy having too much stuff on my rifles (my airsoft M4 being a perfect example. It has as of now dual magazines, retractable stock, foregrip, shotgun-mount, flashlight, laserpointer, holographic sight, x4 no-crosshair scope to go with the holo (I can flip this away with the flick of a switch, allowing for CQB) and a silencer. No games can give me all this!), but still =3

This. Bloody well this! AAAAAAAAH! This is the main thing that pisses me off in every game!

not have random, unprovoked character changes in any character in the game. This is especially heinous when it is done as a dues ex machina.

No cheap kills or jumping around genres. Example: an FPS having platforming sections.

the only thing worse is killing off a main character.
You can kill off the main character if these conditions are fulfilled[footnote]note: this list also applies to lovable side kicks[/footnote]:
A. the death of the character makes sense (I am looking at you Fallout 3)
2. it is actually in character (ties into A but this one is broken more often than not it needs to be stated)
C. it is done awesomely such as a last stand or in a blaze of glory. You must take out a triple digit amount of enemies before or during your death.
D. The conflict must after the main character's death.

games where if you want to open a door you have to press the X button 100 BASTARDING TIMES!!! Why is that necessary???? I mean I can sort of understand it if there are enemies you have to knock back first otherwise they'll hack off your nuts while your doing it, but X-Men Origins: Wolverine would make you do it to open the front door on your way to nipping to the shops if they could. All i'm doing is damaging my controller and my thumb, and it means my cousin who adores X-Men can't play it very much because his cerebal palsy won't let him repeatedly tap a button, so congratulations Activision, you have managed to annoy me and my aunt (who also shares in controller mashing responsibilty), discriminate against a child with a severe disablity and damage game controllers in move fell swoop.

I so agree with 3.

I don't know how many times I've played a game where I have to partner up with an AI for a while, and we have to travel somewhere together, and I am ALWAYS faster than them. What's the deal?
A lot of the time, it seems that a main character can only sprint, so you're left with a bunch of unrealistic stops making it look like you are only operating on red/green traffic lights- no one likes yellow, apparantly.
It can't be that hard, can it, to make the AI just a touch faster?
Geeze.

Escort quests/missions where the person you must escort is so feeble they would die on a Saturday morning stroll round Tescos. By the end of those, I hate the character and want them to die.

Not play the addons to it did you?
There were no levels in that game that weren't used twice. I refuse to pay extra just to get some variety.

The A.I. must not hear the difference between the footsteps of the player or another guard.

40 guards walking patrols, but as soon as you climb out of that vent 'Wait! I thought I heard something. Let's go check it out'

Game Mode Changes.

Both of the Mass Effect games are extremely guilty of this, with their stupid "optional" resource gathering side-games. Other obvious exampels are veichle parts in games that aren't driving games, like Gears of War and Borderlands, stealth sections in action games, action sections in stealth games, etc.

Mini-games can be entertaining if done well, but after my 50th resource gathering in Mass Effect I was pretty bored, not to mention that you'd need to do several hundreds if you were a completionist. And Borderlands veichle sections? Armory of General Knoxx in particular, it was just drive in a straight line for 5 minutes, fighting 5-6 enemies...

I don't play these games to do mini-games that make the apps on my iPhone look like genius-inspired works of brilliance.
