My favorite fail scene has to be the entire series of death movies in Dead Space.
Seriously, when a tentacle jumps at you, eats your face off, attaches itself to yo and walks your body away, you have to just stand back and say "Awesome."

They were probably the best part of the game.

The original Shadowgate is definately on the list of games with the most ways to die. My favs are when you smash the wrong mirror and get sucked into space, or when you try to take the flute without wearing the gauntlet. Acid gets you.

hell yeh the dividers were so awesome, also the lurkers were pretty cool as well, when they grab you you can either A:drop kick a mutated baby and save yourself, or B: let a baby rip off your head
here are the links

Dead Space had so many good death animations.

I like the one in Dead space when you're grabbed by the long arm thing. As my character was pulled into the hole he suddenly reappeared looking healthy as ever, taking a few steps back considering his luck getting out. Before the arm suddenly reappeared and snapped him back into the hole. For a moment I felt hope xD

When you kill your Brother in Red Faction:Geurilla.

"WTF YOU KILLED YOUR BROTHER!?!?!?!?!?!?!?!?!"
This, for sheer comedy value this is the best 'you have died' screen I've seen.

If we're talking about things that have resulted in game over then I would have to say the ridiculously epic last level in Serious Sam: the first encounter (especially if done on Serious or Mental difficulty).

Dead Space- Final Boss.

During the QTE style animation, if you let Isaac get too near it's mouth... well it's just plain nasty. Felt sorry for the poor fella.

Obviously spoilers.....

but I guess my favorite is the ending of Shadow of the Colossus. You finally finish killing the final Colossi, and it ends up that you were helping the bad guy, who subsquently takes over you body and attempts to kill all of the warriors who raided the temple. Sure the bad guy is sealed before the credits, but leaving the main character as a baby......yeah....
you might want to put a spoiler warning on that...

Devil may cry 4. Gun shots are heard and the screen shatters like glass with the phrase "abandon all hope". A paraphrase of what is written over the gate of Hell in Dante's inferno

King's Quest V. It's always hillarious when Graham falls to his death screaming like a little b**ch.

Slo-mo death in Fallout 3, particularly when you have an awesome, explosive death.

And the war quotes when you fall in CoD4.
Those war quotes actually annoyed me for some reason. It was like the game was rubbing it in and it gave me the impression of some really smart guy making an insult about me that was so well executed that I had no idea what it meant only that it felt vaguely insulting. Like he was making a joke that only he got, a confusing and cryptic insult that he knew i wouldn't understand. I knew that wasn't the intended experiance but nonetheless....

As for me, I think the deaths in Dead Space are definetly worth praise, they paid a lot of attention to those.

Man... I wasn'r expecting anyone to know about Total Distortion. Let alone the OP.

But yeah, that one gets my vote. Honorable mentions to the deaths in Clock Tower for the SNES where you run into an elevator, hear screams and then blood leaks out from under the elevator door....creepy shit.
I preferred the one where she is driving away and sees the giant scissors rising slowly from the back seat.
I say the one I just said because you both succeeded and yet failed at the same time.
That death prevented me from sleeping one night. I'm actually not joking. I find it funny THAT was an ending, yet you die, how random

Another one I liked is when that guy eats you if you don't get him some ham. Ahhh, good times.

Oh, yes that death made me laugh. Watch this: http://www.youtube.com/watch?v=QceDCyVZH_E

I refuse to watch it again myself because it disrupts my sleep schedule.
